The term hypomochlion derives from Greek (hypo = under, mochlion = lever) and refers, in medicine and physics, to the fulcrum or pivot point of a lever. It is the fixed point around which a lever arm rotates or at which force is redirected. In anatomy and biomechanics, this concept plays a central role in analyzing human body movement.
Significance in Anatomy and Biomechanics
In the human body, many joints act as hypomochlions. They serve as the fixed pivot points around which muscles and tendons function as levers. The classic example is the elbow joint: when the biceps muscle flexes the forearm, the elbow joint acts as the hypomochlion. Similar mechanisms are found in the knee, shoulder joint, and spine.

- Knee joint: Fulcrum during extension and flexion of the lower leg

Classes of Levers and the Hypomochlion
Biomechanics distinguishes three classes of levers, depending on the position of the hypomochlion relative to the load and the force:
First-Class Lever
The hypomochlion lies between the force and the load. An anatomical example is the atlanto-occipital joint (the joint between the first cervical vertebra and the skull), which serves as the pivot point for nodding the head.
Second-Class Lever
The load lies between the hypomochlion and the force. An example is the metatarsophalangeal joint during toe raises, where body weight is positioned between the ground (pivot) and the calf muscles (force).
Third-Class Lever
The force is applied between the hypomochlion and the load. This lever type is the most common in the human body. A typical example is the biceps muscle acting on the forearm, with the elbow joint as the hypomochlion and the held load at the distal end of the forearm.
Significance in Surgery and Medical Technology
The concept of the hypomochlion is also applied in surgical instrument science. Many surgical instruments such as scissors, clamps, and forceps function according to the lever principle. The hinge of these instruments forms the hypomochlion. The precise positioning of the pivot point significantly affects force transmission and the accuracy of the instrument during use.
The term is also well established in dentistry: during tooth extraction, the contact point of the lever on the alveolar bone acts as the hypomochlion to loosen the tooth from its socket.
Clinical Relevance
Understanding the hypomochlion is essential for:
