What is Heart Failure?
Heart failure (also known as cardiac failure or cardiac insufficiency) is a complex clinical syndrome in which the heart is unable to pump sufficient blood to meet the metabolic demands of the body. As a result, vital organs and tissues receive inadequate oxygen and nutrients. Heart failure can develop suddenly (acute heart failure) or gradually over time (chronic heart failure).
Causes
Heart failure most commonly develops as a consequence of other cardiovascular conditions or chronic diseases. The most frequent causes include:
- Coronary artery disease (CAD): Narrowing of the arteries supplying the heart muscle
- Heart attack (myocardial infarction): Permanent damage to heart muscle tissue
- High blood pressure (hypertension): Long-term overload of the heart
- Heart valve disease: Dysfunctional valves impair normal blood flow
- Cardiomyopathy: Disease of the heart muscle itself
- Arrhythmias: Irregular heartbeat that weakens pumping function
- Diabetes mellitus and obesity: Significantly increase the risk of cardiac damage
Symptoms
Symptoms of heart failure can range from mild to severe depending on the stage of the condition. Common signs include:
- Shortness of breath (dyspnea), especially during exertion or when lying flat
- Persistent fatigue and weakness
- Swelling (edema) in the legs, ankles, and feet due to fluid retention
- Rapid or irregular heartbeat (palpitations)
- Persistent cough or wheezing, often worse at night
- Sudden weight gain from fluid buildup
- Difficulty concentrating and mental confusion
NYHA Classification
The severity of heart failure is commonly assessed using the NYHA (New York Heart Association) classification:
- NYHA Class I: No limitation of physical activity
- NYHA Class II: Slight limitation during moderate exertion
- NYHA Class III: Marked limitation with minimal exertion
- NYHA Class IV: Symptoms at rest; any physical activity causes discomfort
Diagnosis
Heart failure is diagnosed through a combination of clinical evaluation and diagnostic tests, including:
- Physical examination: Listening to the heart and lungs, checking for fluid retention
- Echocardiography: Ultrasound imaging of the heart to assess pumping function (ejection fraction)
- ECG (Electrocardiogram): Measuring the electrical activity of the heart
- Blood tests: Measuring biomarkers such as BNP (B-type natriuretic peptide) or NT-proBNP
- Chest X-ray: Evaluating heart size and signs of lung congestion
- Exercise stress test: Assessing cardiac performance under physical load
Treatment
The goals of heart failure treatment are to relieve symptoms, slow disease progression, reduce hospitalizations, and improve quality of life. Treatment involves medication, lifestyle changes, and in some cases, surgical interventions.
Medication Therapy
- ACE inhibitors or ARBs (angiotensin receptor blockers): Lower blood pressure and reduce cardiac workload
- Beta-blockers: Slow the heart rate and reduce oxygen demand
- Diuretics: Help eliminate excess fluid through the kidneys
- Mineralocorticoid receptor antagonists (e.g., spironolactone): Protect the heart muscle
- SGLT2 inhibitors: A newer class of drugs shown to significantly improve outcomes in heart failure
Lifestyle Modifications
- Low-sodium diet and controlled fluid intake
- Regular, supervised physical activity (cardiac rehabilitation)
- Weight management and avoidance of alcohol and smoking
- Daily monitoring of body weight and blood pressure at home
Interventional and Surgical Options
- Pacemaker or implantable cardioverter-defibrillator (ICD) implantation
- Cardiac resynchronization therapy (CRT): Improves coordination between the heart chambers
- Heart transplantation: For severe, treatment-resistant heart failure
- Mechanical circulatory support (LVAD): Temporary or permanent mechanical pump assistance
Prognosis and Prevention
Heart failure is a chronic condition, but with consistent treatment and a heart-healthy lifestyle, it can be well managed. Early diagnosis is crucial. Controlling risk factors such as high blood pressure, diabetes, and obesity is key to preventing the onset of heart failure. Regular follow-up with a healthcare provider is essential for long-term management.
