What is Macrogol?
Macrogol (also known as polyethylene glycol or PEG) is an osmotic laxative used to treat constipation. It belongs to the class of osmotic laxatives and is available over the counter in various dosage forms, most commonly as a powder that is dissolved in water before ingestion.
Mechanism of Action
Macrogol is a high-molecular-weight polymer that is not absorbed by the intestinal lining. Once in the gut, it binds water molecules through osmotic pressure, increasing the water content of the stool. This softens the stool and increases its bulk, making it easier to pass. Unlike some stimulant laxatives, macrogol acts entirely within the bowel and does not enter the bloodstream.
Indications
- Chronic constipation in adults and children
- Short-term relief of acute constipation
- Bowel preparation before colonoscopy (at higher doses)
- Stool regulation in elderly patients or following surgical procedures
Dosage and Administration
Macrogol is typically supplied as a powder that is dissolved in a glass of water. The standard adult dose is 1 to 3 sachets per day, depending on the severity of constipation. Specially formulated, lower-dose preparations are available for children aged 2 years and older. The effect usually occurs within 24 to 72 hours. Adequate fluid intake during treatment is recommended to support the mechanism of action.
Side Effects
Macrogol is generally well tolerated. Possible side effects include:
- Bloating and flatulence
- Abdominal cramping or discomfort
- Nausea
- Diarrhea if the dose is too high
- Rarely: allergic reactions
Interactions and Contraindications
Macrogol should not be used in the following situations:
- Bowel obstruction (ileus)
- Perforation or inflammation of the gastrointestinal tract
- Known hypersensitivity to macrogol
No significant drug interactions have been reported, as macrogol is not absorbed into the bloodstream. However, because macrogol accelerates intestinal transit, other medications taken at the same time may be transported through the gut more quickly, potentially reducing their absorption. A time interval of at least one hour between macrogol and other medications is generally advised.
Special Considerations
Macrogol is considered one of the best-tolerated laxatives available. It is also regarded as relatively safe for use during pregnancy and breastfeeding, given that it is not absorbed systemically. However, long-term use should be discussed with a healthcare professional. High-dose preparations used for bowel cleansing before colonoscopy should only be administered under medical supervision.
