What is Rivaroxaban?
Rivaroxaban is a direct oral anticoagulant (DOAC) and belongs to the drug class of Factor Xa inhibitors. It is marketed under the brand name Xarelto and is one of the most widely prescribed blood-thinning medications worldwide. Unlike older anticoagulants such as warfarin, rivaroxaban does not require routine blood monitoring (INR testing), making it more convenient for patients.
Indications
Rivaroxaban is approved for several medical indications:
- Prevention of deep vein thrombosis (DVT) following major orthopedic surgery (e.g., hip or knee replacement)
- Treatment and secondary prevention of DVT and pulmonary embolism (PE)
- Stroke prevention in patients with non-valvular atrial fibrillation
- Prevention of heart attack and stroke in patients with coronary artery disease or peripheral artery disease (combined with aspirin)
Mechanism of Action
Rivaroxaban selectively and reversibly inhibits coagulation Factor Xa, which plays a central role in the blood clotting cascade. By blocking Factor Xa, the formation of thrombin (Factor IIa) is reduced, which in turn prevents the conversion of fibrinogen to fibrin and the formation of blood clots (thrombi). Rivaroxaban acts on both free and clot-bound Factor Xa, offering effective anticoagulation.
Dosage
The dosage of rivaroxaban depends on the specific indication:
- DVT/PE treatment: 15 mg twice daily for the first 3 weeks, followed by 20 mg once daily (taken with the main meal)
- Atrial fibrillation / stroke prevention: 20 mg once daily (with the main meal); 15 mg once daily in patients with moderate renal impairment
- DVT prophylaxis after surgery: 10 mg once daily (regardless of meals)
- Secondary prevention after acute coronary syndrome (with aspirin): 2.5 mg twice daily
The exact dosage must always be determined by the prescribing physician, especially in patients with impaired kidney or liver function.
Side Effects
Like all anticoagulants, rivaroxaban increases the risk of bleeding. Common and clinically relevant side effects include:
- Bleeding events (e.g., nosebleeds, gum bleeding, prolonged bleeding from cuts)
- Gastrointestinal bleeding
- Anemia due to chronic blood loss
- Nausea and gastrointestinal discomfort
- Elevated liver enzyme levels (rare)
- Serious internal bleeding (rare but potentially life-threatening)
Patients should seek immediate medical attention if they notice signs of serious bleeding, such as blood in urine, black or tarry stools, or sudden severe headache.
Contraindications and Precautions
Rivaroxaban should not be used in patients with:
- Active, clinically significant bleeding
- Severe liver disease associated with increased bleeding risk
- Pregnancy or breastfeeding
- Concomitant use of certain medications (e.g., strong CYP3A4 and P-gp inhibitors such as azole antifungals or HIV protease inhibitors)
Special caution is required in patients with impaired renal function, as rivaroxaban is partially excreted by the kidneys.
Important Notes for Patients
- Rivaroxaban should be taken regularly and at the same time each day.
- The 15 mg and 20 mg doses must be taken with a meal to ensure adequate absorption.
- Do not stop taking rivaroxaban without consulting your doctor.
- Always inform your doctor or dentist about rivaroxaban use before any surgical or dental procedures.
- There is a specific reversal agent available: Andexanet alfa can be used in cases of serious or life-threatening bleeding.
