What is Umckaloabo?
Umckaloabo is a herbal medicinal product obtained from the root extract of the Cape pelargonium (Pelargonium sidoides). The name originates from the Zulu language and roughly translates to "severe cough." The preparation has a long history in traditional South African medicine and is now available in many countries as a standardized plant extract known as EPs 7630.
Mechanism of Action
The extract of Pelargonium sidoides contains various bioactive compounds, including coumarins, flavonoids, and phenolic acids. These constituents act through several mechanisms:
- Antibacterial: Inhibition of the growth of certain pathogens such as Haemophilus influenzae and Streptococcus pneumoniae.
- Antiviral: Blocking the attachment of viruses to mucosal cells, thereby slowing the spread of respiratory infections.
- Immunomodulatory: Enhancement of immune cell activity (e.g., natural killer cells) and promotion of interferon production.
- Secretolytic: Liquefaction of viscous mucus in the airways, facilitating expectoration.
Indications
Umckaloabo is primarily used for acute respiratory tract infections, including:
- Acute bronchitis
- Common cold (upper respiratory infections)
- Sinusitis (inflammation of the paranasal sinuses)
- Tonsillitis (inflammation of the tonsils)
It is available for both adults and children above certain ages in appropriate dosage forms such as drops and tablets.
Dosage and Usage
Dosage recommendations depend on age and formulation. Common guidelines include:
- Adults: 30 drops three times daily or equivalent tablet form.
- Children (6-12 years): 20 drops three times daily.
- Children (2-5 years): 10 drops three times daily (only after consulting a doctor).
The typical duration of treatment is 7 to 10 days. If symptoms do not improve within 5 days, medical advice should be sought.
Side Effects and Safety
Umckaloabo is generally well tolerated. Possible side effects include:
- Gastrointestinal complaints such as nausea or stomach pain
- Mild allergic reactions (skin rash, itching)
- In very rare cases: elevated liver enzymes (hepatotoxic reactions have been reported)
Individuals with known hypersensitivity to Pelargonium species, as well as patients with severe liver or kidney disease, should not use this preparation. Due to the ethanol content in the drops, caution is advised in patients with alcohol dependency.
Scientific Evidence
Several clinical trials and meta-analyses have investigated the efficacy of Pelargonium sidoides extract (EPs 7630) in acute bronchitis and the common cold. The Cochrane Collaboration assessed the available studies as indicative of superiority over placebo in shortening illness duration and relieving symptoms. The overall level of evidence is considered moderate, and further high-quality studies are desirable.
