What Is Urea Cream?
Urea cream is a medical or cosmetic skin care product containing the active ingredient urea (carbamide). Urea is a substance naturally found in the human body that plays a key role in maintaining skin hydration. Urea creams are available in a wide range of concentrations – from 5% for daily moisturizing care to 40% for the treatment of heavily keratinized skin areas.
Mechanism of Action
Urea works at several levels within the skin:
- Moisture binding (humectancy): Urea draws water from the environment into the stratum corneum (the outermost layer of the skin) and retains it there, keeping the skin soft and hydrated.
- Keratolytic effect: At higher concentrations (from approximately 10%), urea breaks hydrogen bonds in keratin, gently loosening and removing hardened or dead skin cells.
- Barrier function support: Regular use strengthens the skin's natural protective barrier and reduces transepidermal water loss (TEWL).
Indications and Uses
Urea creams are used for a wide range of skin conditions:
- Dry and very dry skin (xerosis cutis): Urea at concentrations of 5–10% is particularly effective as a base therapy for dry skin.
- Psoriasis: Urea helps loosen scales and makes the skin more supple.
- Ichthyosis: In this genetically caused keratinization disorder, urea is a central component of long-term therapy.
- Diabetic foot syndrome: Urea creams (typically 10–25%) are used to care for and prevent cracks and calluses on the feet.
- Nail fungus treatment: In combination products, 40% urea is used to soften and remove diseased nail tissue.
- Atopic dermatitis (eczema): Low-concentration urea creams (5%) support basic skin care in sensitive skin conditions.
- Calluses on hands and feet: Medium to high concentrations (20–40%) provide callus-softening and regenerative effects.
Concentration Ranges and Their Effects
- 2–5% urea: Gentle moisturizing care, suitable for sensitive skin, children, and facial use.
- 10% urea: Intensive hydration with mild keratolytic action, ideal for body, hands, and feet.
- 20–25% urea: Strongly moisturizing and callus-softening, especially for thickened skin on the feet.
- 40% urea: Pronounced keratolytic effect, used specifically for thickened nails or heavily keratinized skin areas, usually under medical supervision.
Application and Dosage
Urea creams are typically applied once or twice daily in a thin layer to the affected skin areas. For facial application or use in children, low-concentration formulations (under 5%) are recommended. Urea creams should not be applied to broken, inflamed, or open skin, as this may cause burning or irritation.
Side Effects and Tolerability
Urea creams are generally well tolerated. Possible side effects include:
- Mild burning or tingling immediately after application, especially with higher concentrations
- Skin irritation in very sensitive or inflamed skin
- Rarely, allergic reactions
Pregnant and breastfeeding women should seek medical advice before using high-concentration urea creams.
