A vaccination certificate (also called an immunization certificate or proof of vaccination) is an official document confirming that an individual has received one or more vaccinations. Vaccination certificates can be issued in paper form or digitally.

Traditional Vaccination Booklet
The traditional vaccination booklet is a small paper booklet in which all administered vaccines are recorded by hand or stamp. In Germany, the yellow vaccination booklet issued by the Robert Koch Institute (RKI) is widely used. It includes entries for standard vaccinations such as tetanus, measles, and influenza.
Digital Vaccination Certificate
During the COVID-19 pandemic, the European Union introduced the EU Digital COVID Certificate. This digital document could be stored via QR code on a smartphone and presented when required. It was tamper-proof and recognised across EU borders. Similar digital solutions exist worldwide.

A complete vaccination certificate typically contains the following information:

- Occupational requirements: In healthcare professions or when working with vulnerable populations, specific vaccination records may be legally required.
- Schools and childcare: For children entering care facilities, proof of vaccination (e.g., against measles) may be legally mandated.
- Personal health management: The certificate helps physicians and patients track vaccination status and plan booster doses in a timely manner.
Legal Framework
In Germany, the Infection Protection Act (Infektionsschutzgesetz, IfSG) provides the legal basis for vaccination records. Since 2020, proof of measles vaccination has been legally required for children in communal facilities and for certain professional groups. Vaccination certificates are issued by licensed physicians or authorised vaccination centres.
Loss and Renewal
If a vaccination booklet is lost, vaccination records can in many cases be re-issued retroactively by the treating physician or public health office, provided the vaccinations were documented. It is therefore advisable to keep a copy of the vaccination booklet. In some cases, missing vaccination records can be supplemented by antibody testing, which indirectly confirms immune protection.
