What is Vein Stripping?
Vein stripping is an established surgical procedure used to remove superficial, diseased veins, most often in the context of significant varicose veins (varices). The affected vein is pulled out of the tissue using a specially designed surgical probe. The procedure is performed in vascular surgery and phlebology and is one of the most common venous surgeries worldwide.
When is Vein Stripping Performed?
The procedure is indicated when varicose veins cause considerable discomfort or when complications are likely. Typical indications include:
- Significant trunk varicosis of the great saphenous vein or small saphenous vein
- Chronic venous insufficiency with swelling, pain, and skin changes
- Recurrent superficial vein inflammation (thrombophlebitis)
- Venous leg ulcers caused by venous congestion
- Considerable cosmetic impairment affecting quality of life
How the Procedure Works
Vein stripping is usually performed under spinal or general anesthesia. The surgeon makes small incisions at the groin and lower leg. A flexible wire probe is inserted into the lumen of the vein and secured at its tip. The probe is then pulled through and out, removing the vein along with it – this is the actual stripping step. Smaller tributary branches are removed through tiny stab incisions (phlebectomy). A compression bandage is applied after surgery.
Variants of the Procedure
- Babcock stripping: The classic technique in which the entire vein is pulled out.
- Short stripping: Only the upper segment of the vein is removed to reduce the risk of nerve injury.
- Invagination stripping (PIN stripping): The vein is inverted inside itself before removal, causing less tissue trauma.
Preparation and Aftercare
Before surgery, a duplex ultrasound examination is performed to map the exact course and extent of the diseased veins. Blood-thinning medications may need to be paused beforehand. After the operation, wearing compression stockings and walking regularly are essential to minimize the risk of deep vein thrombosis. Physical rest is recommended for several weeks.
Risks and Possible Complications
As with any surgical procedure, complications can occur, although they are generally uncommon:
- Bruising (hematomas) along the surgical path
- Wound infections or impaired wound healing
- Nerve injuries leading to temporary or, rarely, permanent numbness
- Scarring at incision sites
- Deep vein thrombosis (rare)
- Recurrence of varicose veins
Alternatives to Vein Stripping
Modern minimally invasive techniques may be suitable alternatives in many cases:
- Endovenous laser ablation (EVLA): Closure of the vein using laser energy
- Radiofrequency ablation (RFA): Thermal closure using high-frequency electrical current
- Foam sclerotherapy: Chemical destruction of the vein using a foam agent
- Mechanochemical ablation (MOCA): Combination of mechanical and chemical action
The most appropriate treatment is determined by the clinical findings, the extent of the disease, and the individual situation of the patient, and should be decided together with a specialist in phlebology or vascular surgery.
