What Is a Hospital Admission Referral?
A hospital admission referral (German: Einweisung) is an official medical document issued by a licensed physician that authorizes a patient to be admitted to a hospital, specialist clinic, or psychiatric facility for inpatient care. It serves as the formal basis for the patient to receive inpatient treatment and is required by health insurance providers -- both statutory and private -- as justification for covering the costs of the hospital stay.
Types of Hospital Admission Referrals
- Elective (planned) referral: The hospital stay is scheduled in advance, for example for a surgical procedure or a diagnostic workup.
- Emergency referral: Issued immediately in cases of acute, life-threatening, or urgent medical conditions -- often by an emergency physician or the on-call medical service.
- Psychiatric referral: Admission to a psychiatric facility, which in certain cases may be involuntary (compulsory admission) when there is a risk to the patient or others.
Process and Content
The treating physician completes a referral form (in Germany, this is the standardized Form Muster 2 for statutory health insurance patients). This document typically includes:
- Patient details (name, date of birth, insurance number)
- Diagnosis or suspected diagnosis (usually coded using ICD-10)
- Reason and urgency of admission
- Recommended hospital or specialist department
- Stamp and signature of the referring physician
When Is a Hospital Admission Referral Necessary?
A referral is issued when outpatient treatment is no longer sufficient and inpatient medical care is required. Common reasons include:
- Serious illnesses or injuries requiring continuous monitoring
- Surgical or invasive procedures requiring a hospital stay
- Diagnostic investigations that cannot be performed on an outpatient basis
- Acute psychiatric crises or mental health conditions
- Complications arising from chronic diseases
Patient Rights
Patients generally have the right to participate in choosing the hospital they are referred to, provided this is medically appropriate. In the case of a compulsory admission (regulated under the respective German state mental health laws, known as PsychKG), strict legal requirements and procedural safeguards apply to protect the rights of the individual. Patients who are deemed capable of giving informed consent may also refuse a planned hospital admission.
Referral vs. Specialist Referral
A hospital admission referral always relates to inpatient admission to a hospital. A specialist referral, by contrast, directs the patient to another physician or specialist for outpatient consultation. Both are formal medical documents but differ in their purpose and setting.
