There are three main subtypes:
- Neutrophils: the most common subtype, making up approximately 50–70% of granulocytes; primarily fight bacterial infections

The absolute granulocyte count refers to the actual number of granulocytes per microliter (µl) of blood. Unlike the relative (percentage) value, the absolute count provides a more precise picture of the true quantity of these immune cells in circulation.
In clinical practice, the Absolute Neutrophil Count (ANC) is the most commonly used measure, as neutrophils represent the largest granulocyte subgroup and are central to fighting infections.
Reference Values
Normal ranges may vary slightly between laboratories. Generally accepted reference values for adults are:
- Total granulocytes (absolute): approx. 1,800 – 7,500 cells/µl
- Neutrophils (absolute / ANC): approx. 1,800 – 7,500 cells/µl
- Eosinophils (absolute): approx. 50 – 500 cells/µl
- Basophils (absolute): approx. 0 – 100 cells/µl
Causes of Elevated Granulocyte Counts (Granulocytosis)
An elevated absolute granulocyte count is called granulocytosis and may indicate:
- Bacterial infections
- Inflammatory conditions (e.g., rheumatoid arthritis)
- Tissue injury or recent surgery
- Certain medications (e.g., corticosteroids)
- Physical stress or strenuous exercise
- Certain blood cancers (e.g., chronic myeloid leukemia)
Causes of Low Granulocyte Counts (Granulocytopenia / Agranulocytosis)
A reduced absolute granulocyte count is called granulocytopenia. A severe drop is referred to as agranulocytosis. Possible causes include:
- Viral infections (e.g., influenza, HIV)
- Chemotherapy or radiation therapy
- Certain medications (e.g., antithyroid drugs, clozapine)
- Autoimmune disorders
- Bone marrow diseases (e.g., aplastic anemia)
- Severe sepsis
Clinical Significance and Diagnosis
The absolute granulocyte count is determined as part of a differential blood count, which breaks down the various subtypes of white blood cells. This test is used when infections, immune deficiencies, or blood disorders are suspected, and for monitoring patients undergoing immunosuppressive therapy.
Monitoring the ANC is especially important for patients receiving chemotherapy. An ANC below 500 cells/µl indicates severe neutropenia and significantly increases the risk of life-threatening infections.
Treatment for Abnormal Values
Treatment always targets the underlying cause:
- For bacterial infections: antibiotic therapy
- For chemotherapy-induced granulocytopenia: administration of G-CSF (Granulocyte Colony-Stimulating Factor) to stimulate bone marrow production
- For drug-induced agranulocytosis: immediate discontinuation of the causative medication
- For autoimmune causes: immunosuppressive therapy
