What is Aneurysm Clipping?
Aneurysm clipping is a neurosurgical technique used to treat intracranial (brain) aneurysms. A brain aneurysm is an abnormal bulge or ballooning in the wall of a cerebral artery. If left untreated, it can rupture and cause a subarachnoid hemorrhage -- a life-threatening bleed into the space surrounding the brain. During clipping, a small metal clip is placed at the neck of the aneurysm to cut off blood flow into the bulge and prevent rupture.
When is Aneurysm Clipping Used?
The procedure is indicated in two main situations:
- Elective (preventive): When an unruptured aneurysm is detected and is considered at high risk of rupture due to its size, shape, or location.
- Emergency: After a ruptured aneurysm has already caused a subarachnoid hemorrhage, to prevent re-bleeding.
How Does Aneurysm Clipping Work?
The procedure is performed under general anesthesia and requires a craniotomy (opening of the skull). The neurosurgeon carefully navigates to the affected artery and places one or more precisely shaped titanium clips at the neck of the aneurysm. This stops blood from entering the bulge while preserving normal blood flow through the artery. The clip remains in the body permanently and is generally compatible with MRI scans.
Diagnosis and Pre-operative Planning
Before surgery, imaging techniques are used to map the aneurysm and plan the surgical approach:
- CT Angiography (CTA): Rapid imaging of cerebral vessels using contrast dye.
- Magnetic Resonance Angiography (MRA): Non-radiation-based vessel imaging.
- Digital Subtraction Angiography (DSA): The gold standard for detailed visualization of brain aneurysms.
Alternative Treatment: Coiling
A less invasive alternative to clipping is endovascular coiling. In this procedure, platinum coils are inserted through a catheter into the aneurysm to fill and seal it from the inside. The choice between clipping and coiling depends on the anatomy and location of the aneurysm, the age and overall health of the patient, and the expertise of the medical team.
Risks and Complications
As with any neurosurgical procedure, aneurysm clipping carries certain risks, including:
- Intraoperative or postoperative bleeding
- Brain swelling (cerebral edema)
- Infection or meningitis
- Stroke or neurological deficits (e.g., paralysis, speech disorders)
- Misplacement of the clip
- Anesthesia-related complications
The overall risk profile depends strongly on the condition of the patient, the size and location of the aneurysm, and whether the procedure is elective or performed as emergency surgery.
Recovery and Rehabilitation
After clipping, patients typically spend several days to weeks in a neurological intensive care unit. Regular follow-up imaging is needed to monitor the clip and cerebral blood flow. If neurological deficits are present, rehabilitation programs such as physiotherapy, occupational therapy, and speech therapy may be required. With a successful procedure and no major complications, the long-term prognosis is often favorable.
