What Is a Hormone Profile?
A hormone profile (also called a hormonal panel or hormone status test) is a laboratory examination in which the concentrations of various hormones are measured in blood, saliva, or urine. Hormones are chemical messengers produced by the body that regulate a wide range of vital functions – including metabolism, growth, sleep, mood, fertility, and immune response. A hormone profile gives healthcare providers a comprehensive overview of a patient´s hormonal balance.
Which Hormones Are Tested?
Depending on the clinical question, different hormone groups may be assessed. Common parameters include:
- Thyroid hormones: TSH, free T3, free T4 – to evaluate thyroid function
- Sex hormones: estrogen, progesterone, testosterone, LH, FSH – relevant for fertility issues, menstrual irregularities, or menopausal symptoms
- Stress hormones: cortisol, DHEA – to assess adrenal gland function
- Growth hormones: IGF-1, growth hormone (GH) – when growth disorders are suspected
- Metabolic hormones: insulin, blood glucose, leptin – for metabolic conditions
- Prolactin: relevant in cases of infertility or unexplained milk production outside of breastfeeding
When Is a Hormone Profile Recommended?
A hormone profile is ordered by doctors in a variety of clinical situations. Common indications include:
- Unfulfilled desire to have children in women or men
- Irregular or absent menstrual cycles
- Symptoms of menopause or andropause
- Suspected thyroid conditions (e.g., Hashimoto thyroiditis, Graves disease)
- Unexplained weight gain or weight loss
- Chronic fatigue, low motivation, and mood swings
- Acne, hair loss, or excessive body hair growth
- Suspected polycystic ovary syndrome (PCOS)
- Monitoring of an existing hormone therapy
Diagnosis and Procedure
Samples for a hormone profile are usually collected through a simple blood draw, though saliva or 24-hour urine samples may also be used in certain cases. Because many hormone levels fluctuate throughout the day or depend on the menstrual cycle phase, the timing of sample collection is critical for accurate results. For example, cortisol should ideally be measured in the morning, while sex hormones in women are best measured on a specific day of the menstrual cycle. The treating physician will provide detailed preparation instructions.
Reference Values
Laboratory results are interpreted against reference ranges that vary by age, sex, and cycle phase. Deviations from these ranges may indicate hormonal disorders, but must always be assessed in the context of the patient´s overall clinical picture and symptoms.
Treatment of Hormonal Imbalances
If a hormone profile reveals values outside the normal range, treatment depends on the underlying cause:
- Hypothyroidism: thyroid hormone replacement therapy (levothyroxine)
- Estrogen deficiency during menopause: hormone replacement therapy (HRT) after careful benefit-risk assessment
- Testosterone deficiency: testosterone replacement therapy in men
- PCOS: lifestyle changes, medications such as metformin or oral contraceptives
- Adrenal insufficiency: cortisone replacement therapy
In many cases, lifestyle adjustments such as a balanced diet, regular physical activity, stress reduction, and adequate sleep can also positively influence hormonal balance.
