What Is Urine Sediment?
Urine sediment refers to the solid components that settle at the bottom of a urine sample after centrifugation. These components are then examined under a microscope. The analysis of urine sediment is a fundamental part of urinalysis and provides physicians with valuable information about the condition of the kidneys, bladder, and urinary tract.
How Is the Test Performed?
The test typically requires a midstream urine sample, meaning the middle portion of urine collected during urination. This is collected in a sterile container and sent to the laboratory as quickly as possible. There, the sample is centrifuged, the supernatant liquid is removed, and the remaining sediment is placed on a glass slide and examined under a microscope.
What Is Examined in Urine Sediment?
Various components can be identified in urine sediment, each carrying diagnostic significance:
Cellular Components
- Erythrocytes (red blood cells): An elevated count may indicate bleeding in the urinary tract, kidney inflammation, or kidney stones.
- Leukocytes (white blood cells): An increase often points to an infection or inflammation of the urinary tract or kidneys.
- Epithelial cells: These may originate from the bladder, urinary tract, or renal tubules and can indicate inflammation or tissue damage.
Casts
Casts are cylindrical structures that form within the kidney tubules. Several types can be identified:
- Hyaline casts: Can be found in healthy individuals, especially after exercise.
- Granular casts: May indicate kidney disease.
- Red blood cell casts: A strong indicator of glomerulonephritis (inflammation of the kidney filtering units).
- White blood cell casts: Typically associated with pyelonephritis (kidney infection).
Crystals
Various crystal types can be detected in urine sediment, including oxalate, uric acid, and phosphate crystals. These may indicate metabolic disorders or a predisposition to kidney stone formation.
Bacteria and Fungi
The presence of bacteria in urine sediment may suggest a urinary tract infection, but must be interpreted alongside clinical findings and a urine culture. Fungi such as yeast cells can also be detected in certain conditions.
Clinical Relevance
Urine sediment analysis is an important diagnostic tool for the following conditions:
- Urinary tract infections (e.g., cystitis, pyelonephritis)
- Glomerulonephritis (inflammation of the kidney glomeruli)
- Nephrolithiasis (kidney stones)
- Diabetic nephropathy (kidney damage caused by diabetes)
- Systemic diseases involving the kidneys (e.g., lupus erythematosus)
Normal Findings
In a healthy individual, only very few cellular components are found in urine. Normal reference values generally include:
- Fewer than 5 erythrocytes per high-power field
- Fewer than 5 leukocytes per high-power field
- Occasional epithelial cells
- No or only occasional hyaline casts
- No bacteria
