What is Venlafaxine?
Venlafaxine is a prescription medication belonging to the class of Serotonin-Norepinephrine Reuptake Inhibitors (SNRIs). It is one of the most widely prescribed antidepressants globally and has been used in clinical practice since the 1990s. The drug is available in several formulations, including immediate-release tablets and extended-release capsules (XR/ER) that provide a slower, more controlled delivery of the active ingredient.
Indications
Venlafaxine is approved for the treatment of the following conditions:
- Major Depressive Disorder (MDD)
- Generalized Anxiety Disorder (GAD)
- Social Anxiety Disorder (social phobia)
- Panic Disorder with or without agoraphobia
In clinical practice, venlafaxine is also used off-label for chronic pain syndromes, fibromyalgia, and the management of vasomotor symptoms (hot flashes) associated with menopause, though these uses may not be officially approved in all countries.
Mechanism of Action
Venlafaxine works by blocking the reuptake of serotonin and norepinephrine into presynaptic nerve cells in the brain. This increases the concentration of these neurotransmitters in the synaptic cleft, enhancing communication between nerve cells. At higher doses, venlafaxine also weakly inhibits the reuptake of dopamine. This dual mechanism distinguishes it from selective serotonin reuptake inhibitors (SSRIs) and may benefit patients who do not respond adequately to SSRI therapy.
Dosage and Administration
The appropriate dose of venlafaxine is determined individually by a healthcare provider. Common dosage ranges include:
- Depression: 75–225 mg per day
- Anxiety disorders: 75–225 mg per day
- Starting dose: typically 37.5 mg or 75 mg per day, with gradual increases as needed
The extended-release formulation is taken once daily and generally offers better tolerability compared to the immediate-release form. Venlafaxine should always be taken with food. The full antidepressant effect typically becomes apparent after 2–4 weeks of consistent use.
Side Effects
Like all medications, venlafaxine can cause side effects. Commonly reported side effects include:
- Nausea, especially at the start of treatment
- Insomnia or drowsiness
- Dizziness and headache
- Dry mouth
- Elevated blood pressure, particularly at higher doses
- Sexual dysfunction
- Increased sweating
Less common but more serious side effects include serotonin syndrome — a potentially life-threatening condition that can occur when venlafaxine is combined with other serotonergic substances — and cardiac arrhythmias. Abrupt discontinuation can lead to a discontinuation syndrome, characterized by dizziness, nausea, and flu-like symptoms. For this reason, venlafaxine should always be tapered gradually under medical supervision.
Important Warnings and Contraindications
Venlafaxine must not be used together with monoamine oxidase inhibitors (MAOIs), as this combination can cause life-threatening reactions. Caution is required in patients with:
- Pre-existing high blood pressure
- Heart disease
- Epilepsy
- Pregnancy and breastfeeding
- Children and adolescents under 18 years of age
In young adults, venlafaxine may increase the risk of suicidal thoughts, particularly at the beginning of treatment. Patients and caregivers should monitor for changes in mood or behavior and seek immediate medical attention if any concerns arise.
