What is Bacteria in the Urine?
Bacteria in the urine, medically referred to as bacteriuria, means that detectable amounts of bacteria are present in a person's urine. Healthy urine is normally sterile. When bacteria are detected, it may indicate an infection of the urinary tract -- though it does not always cause noticeable symptoms.
A distinction is made between symptomatic bacteriuria, where symptoms such as a burning sensation during urination are present, and asymptomatic bacteriuria, where bacteria are found in the urine without any symptoms.
Causes
Bacteria most commonly enter the urinary tract through the urethra and multiply there. Common causes include:
- Ascending infections caused by intestinal bacteria, most frequently Escherichia coli (E. coli)
- Poor intimate hygiene or incorrect wiping direction (back to front)
- Use of urinary catheters (catheter-associated urinary tract infections)
- Underlying health conditions such as diabetes mellitus or a weakened immune system
- Anatomical factors, such as the shorter urethra in women
- Urinary obstruction caused by kidney stones or an enlarged prostate
Symptoms
Not all cases of bacteriuria cause symptoms. When symptoms do occur, they may include:
- Burning or pain during urination (dysuria)
- Frequent urge to urinate with only small amounts of urine passed
- Cloudy or foul-smelling urine
- Blood in the urine (haematuria)
- Pressure or pain in the lower abdomen
- In cases of an ascending infection (pyelonephritis): fever, chills, and back or flank pain
Diagnosis
Diagnosis is typically made through a urine examination. The following methods are used:
- Urine dipstick test (rapid test): Detects leucocytes (white blood cells) and nitrite, which can indicate a bacterial infection.
- Urine sediment analysis: Microscopic examination of the urine for bacteria and cells.
- Urine culture (midstream urine): Laboratory cultivation of bacteria to identify the exact pathogen and determine antibiotic sensitivity (antibiogram).
A correctly collected urine sample is essential for reliable results. The midstream urine sample -- collected after cleaning the genital area and discarding the first stream of urine -- is the standard method to avoid contamination.
Treatment
Treatment depends on whether symptoms are present and the specific patient group involved:
Symptomatic Urinary Tract Infections
When symptoms are present, an antibiotic treatment is usually prescribed. Commonly used antibiotics include nitrofurantoin, fosfomycin, and trimethoprim. The choice of antibiotic is guided by the antibiogram and local resistance patterns.
Asymptomatic Bacteriuria
In healthy adults without symptoms, treatment is generally not required. Exceptions include pregnant women and patients scheduled for urological procedures, as the risk of complications is higher in these groups.
General Measures
- Stay well hydrated (at least 1.5 to 2 litres of water per day)
- Maintain proper intimate hygiene
- Review contraceptive methods if diaphragms or spermicides are used
- For recurrent infections: consider long-term prophylaxis or herbal remedies (e.g. cranberry extract) in consultation with a doctor
