What Is the Baldwin Sign?
The Baldwin sign is a clinical examination finding used during the physical assessment of patients with suspected renal vein thrombosis (thrombosis of the vena renalis). It describes a characteristic tenderness upon palpation of the flank or costovertebral angle -- the area between the lowest rib and the spine. The sign is named after the physician who first described it as a diagnostic indicator.
Clinical Background
Renal vein thrombosis is a rare but potentially serious condition in which a blood clot forms within the renal vein. It can follow an acute or chronic course and is often associated with nephrotic syndrome, tumors, trauma, or coagulation disorders. Early diagnosis is essential to prevent complications such as pulmonary embolism.
How the Examination Is Performed
To assess the Baldwin sign, the patient is placed in a supine or lateral position. The examiner applies targeted pressure to the costovertebral angle on the affected side. The sign is considered positive when this pressure elicits a noticeable pain response in the flank region.
Symptoms and Associated Findings
In addition to a positive Baldwin sign, renal vein thrombosis may present with the following symptoms:
- Flank pain: A dull or sharp pain in the lateral back or abdominal area
- Hematuria: Blood in the urine, either visible or detected microscopically
- Proteinuria: Elevated protein levels in the urine
- Edema: Swelling, particularly when both kidneys are affected
- Nausea and vomiting: Especially in acute presentations
Diagnosis
The Baldwin sign serves as an indicator and does not replace imaging or laboratory diagnostics. The following investigations are used to confirm renal vein thrombosis:
- Doppler ultrasound: Assesses blood flow within the renal vein
- CT angiography: Contrast-enhanced imaging for precise visualization of vascular anatomy and the thrombus
- MRI: A radiation-free alternative for detailed soft tissue imaging
- Laboratory tests: D-dimers, coagulation parameters, renal function markers, and urinalysis
Treatment
Management of renal vein thrombosis depends on the underlying cause, the extent of the clot, and the clinical course:
- Anticoagulation: Inhibiting blood clotting with heparin or oral anticoagulants (e.g., vitamin K antagonists or direct oral anticoagulants)
- Thrombolysis: In severe acute cases, pharmacological dissolution of the clot may be considered
- Treatment of the underlying condition: Addressing the root cause such as nephrotic syndrome or a tumor
