What is Betaine HCl?
Betaine HCl (betaine hydrochloride) is a compound formed from betaine – a naturally occurring derivative of the amino acid glycine – and hydrochloric acid (HCl). As a dietary supplement, it is primarily used to increase stomach acid levels and improve digestion. In integrative and functional medicine, it is considered a go-to option for managing hypochlorhydria (insufficient stomach acid production).
Mechanism of Action
Betaine HCl lowers the pH in the stomach by releasing hydrochloric acid when dissolved in gastric fluid. This supports several key physiological processes:
- Activation of pepsinogen into active pepsin, the primary protein-digesting enzyme in the stomach
- Optimization of protein digestion and breakdown
- Improved absorption of minerals such as iron, zinc, calcium, and magnesium, as an acidic environment enhances their solubility
- Antimicrobial effect against pathogens ingested with food
- Stimulation of digestive enzyme release from the pancreas
Medical Applications
Hypochlorhydria and Achlorhydria
Hypochlorhydria (reduced stomach acid) or achlorhydria (complete absence of stomach acid) can significantly impair digestion. Symptoms may include bloating, belching, a feeling of fullness after meals, poor nutrient absorption, and food intolerances. Betaine HCl can substitute for the missing acid in these cases.
Protein Digestion and Nutrient Absorption
Adequate stomach acid production is essential for optimal protein digestion and the absorption of micronutrients. Betaine HCl is therefore used as a supplement in cases of nutrient deficiencies and digestive disorders.
Betaine as an Independent Substance
Independently of the HCl component, betaine itself has important biological functions. It acts as a methyl group donor in homocysteine metabolism and may help reduce elevated homocysteine levels, which is associated with a lower risk of cardiovascular disease.
Dosage and Usage
Betaine HCl is typically available in capsule or tablet form and is taken immediately before or with a protein-rich meal. Common dosages range from 325 mg to 650 mg per meal, but may vary depending on individual tolerance and medical advice. Any increase in dosage should be done gradually and under supervision.

Side Effects and Safety Notes

Contraindications: Betaine HCl should not be taken in cases of existing stomach ulcers (peptic ulcer disease), gastritis, acid reflux, concurrent use of non-steroidal anti-inflammatory drugs (NSAIDs) or corticosteroids, or known hypersensitivity to the compound.
Interactions with Other Substances
Betaine HCl may affect the absorption of certain medications by altering gastric pH. In particular, acid-sensitive medications or those with enteric coatings may be affected in terms of efficacy. It is therefore advisable to consult a healthcare professional before starting supplementation.
