The Four CAGE Questions
Patients are asked the following four questions:
- C (Cut down): Have you ever felt you should cut down on your drinking?
- A (Annoyed): Have you ever been annoyed by people criticizing your drinking?
- G (Guilty): Have you ever felt guilty or bad about your drinking?
- E (Eye-opener): Have you ever had a drink first thing in the morning to steady your nerves or get the day started?
Scoring and Interpretation
Each Yes answer scores one point. The total score ranges from 0 to 4:
- 0 points: No indication of problematic alcohol use.
- 1 point: Possible indication; further assessment is recommended.
- 2 or more points: Clinically significant indication of alcohol misuse or dependence; a more thorough diagnostic evaluation is strongly recommended.
A threshold score of 2 or more is widely regarded in clinical literature as a positive screening result, indicating potentially harmful alcohol use.
Clinical Applications
The CAGE Test is commonly used in the following medical settings:

- Psychiatric and psychosomatic facilities
- Emergency departments and inpatient admissions
