What is Candesartan?
Candesartan is a prescription medication belonging to the class of angiotensin II receptor blockers (ARBs), also known as sartans. It is primarily used to treat high blood pressure (hypertension) and heart failure. The drug works by blocking the effects of angiotensin II, a hormone that narrows blood vessels and raises blood pressure, thereby helping to relax the vessels and lower blood pressure.
Common Side Effects of Candesartan
Most patients tolerate candesartan well. However, the following side effects may occur and have been observed in more than 1 in 100 users:
- Dizziness and lightheadedness, especially at the start of treatment
- Headaches
- Low blood pressure (hypotension), particularly when standing up (orthostatic hypotension)
- Back and joint pain
- Mild fatigue and weakness

Rare but Serious Side Effects

- Angioedema: Swelling of the face, lips, tongue, or throat – a medical emergency
- Acute kidney dysfunction or kidney failure
- Severely low blood pressure when taken alongside diuretics (water tablets)
- Elevated liver enzyme levels (very rare)
- Changes in blood count (e.g., leukopenia, thrombocytopenia)
- Severe skin reactions (very rare)
Candesartan Compared to ACE Inhibitors
Unlike ACE inhibitors (e.g., ramipril), candesartan generally does not cause a dry cough, as it does not increase bradykinin levels. This makes it a preferred alternative for patients who cannot tolerate ACE inhibitors due to coughing.
Interactions with Other Medications
Candesartan can interact with various medications, potentially increasing the risk of side effects:
- Potassium-sparing diuretics or potassium supplements: Increased risk of hyperkalemia
- Non-steroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen or diclofenac: May reduce the blood pressure-lowering effect and put strain on the kidneys
- Lithium: Increased risk of lithium toxicity
- Other blood pressure-lowering medications: Enhanced antihypertensive effect
Who Should Not Take Candesartan?
Candesartan is contraindicated (must not be used) in:
- Pregnancy: Candesartan can harm the unborn child and is strictly contraindicated during the 2nd and 3rd trimesters
- Severe liver impairment
- Known hypersensitivity to the active ingredient
- Concomitant use of aliskiren in patients with diabetes or impaired kidney function
