Mechanism of Action
Cephalosporins work by inhibiting bacterial cell wall synthesis. They bind to penicillin-binding proteins (PBPs), which are essential for the construction and stabilization of the bacterial cell wall. This binding disrupts cell wall synthesis, ultimately leading to bacterial cell death. Because human cells do not have a cell wall, this mechanism is highly specific to bacteria.
Generations and Spectrum of Activity
Cephalosporins are classified by generation, each differing in their spectrum of activity:
- 1st Generation (e.g., cefalexin, cefazolin): Primarily effective against gram-positive bacteria; commonly used for skin and soft tissue infections and perioperative prophylaxis.
- 2nd Generation (e.g., cefuroxime, cefaclor): Extended coverage against gram-negative bacteria; used for respiratory and urinary tract infections.
- 3rd Generation (e.g., ceftriaxone, cefotaxime): Highly effective against gram-negative pathogens; used in serious infections such as meningitis and sepsis.
- 4th Generation (e.g., cefepime): Broad spectrum covering both gram-positive and gram-negative bacteria, including certain resistant strains.
- 5th Generation (e.g., ceftaroline): Effective even against methicillin-resistant Staphylococcus aureus (MRSA).
Dosage
The dosage of cephalosporins varies depending on the specific drug, the indication, the severity of the infection, and the renal function of the patient. Some cephalosporins can be taken orally, while others must be administered intravenously or intramuscularly. The exact dosage should always be determined by a qualified healthcare professional.
Side Effects
Cephalosporins are generally well tolerated. Possible side effects include:
- Gastrointestinal complaints (nausea, diarrhea, vomiting)
- Allergic reactions (rash, itching; rarely anaphylactic shock)
- Cross-reactivity with penicillins (in approximately 1-2% of cases)
- Elevated liver enzyme levels
- Disruption of gut flora, and in rare cases Clostridioides difficile infection
Contraindications and Interactions
Cephalosporins should not be used in patients with known hypersensitivity to cephalosporins. Caution is advised in patients with known penicillin allergy. Interactions may occur with anticoagulants (blood thinners) and certain nephrotoxic substances. Dose adjustment is often required in patients with renal impairment.
