What is Chlamydia?
Chlamydia (Chlamydia trachomatis) is a bacterial sexually transmitted infection (STI) and one of the most common STIs worldwide. It primarily affects sexually active women, especially those between the ages of 15 and 25. The infection is transmitted through unprotected vaginal, anal, or oral sex.
Symptoms in Women
One of the most challenging aspects of chlamydia is that approximately 70-80% of infected women experience no symptoms at all. This means the infection often goes undetected and, if left untreated, can lead to serious health complications.
Common Symptoms
- Unusual or increased vaginal discharge (often yellowish or whitish)
- Burning sensation or pain during urination (similar to a urinary tract infection)
- Pelvic or lower abdominal pain
- Spotting between periods or bleeding after sexual intercourse
- Pain during sexual intercourse (dyspareunia)
- Redness or inflammation of the cervix (cervicitis)
Less Common Symptoms
- Rectal pain or discharge (in cases of rectal infection)
- Conjunctivitis (pink eye) if infected secretions come into contact with the eye
- Sore throat in cases of pharyngeal infection
Possible Complications
If left untreated, chlamydia can spread to the upper reproductive tract and cause Pelvic Inflammatory Disease (PID). This can lead to serious long-term consequences, including:
- Inflammation of the fallopian tubes (salpingitis), increasing the risk of ectopic pregnancy
- Infertility due to scarring of the fallopian tubes
- Chronic pelvic pain
- Transmission to a newborn during childbirth, potentially causing neonatal conjunctivitis or pneumonia
Diagnosis
Since many women do not develop symptoms, regular testing is strongly recommended. Chlamydia is diagnosed through:
- Cervical or vaginal swab (can be self-collected)
- Urine sample (first-catch urine)
- Laboratory analysis using a PCR test (Polymerase Chain Reaction) to detect the genetic material of the bacteria
Routine chlamydia screening is recommended for all sexually active women under 25 and for older women with new or multiple partners.
Treatment
Chlamydia is easily treated with antibiotics. Standard treatment options include:
- Azithromycin: Single oral dose of 1 g
- Doxycycline: 100 mg twice daily for 7 days (often the preferred regimen)
It is essential that all sexual partners are also tested and treated if positive to prevent reinfection (the so-called ping-pong effect). Sexual activity should be avoided until both partners have completed treatment.
Prevention
- Consistent use of condoms during sexual activity
- Regular STI testing, especially with new or multiple partners
- Open communication with sexual partners about STI status
- Participation in routine chlamydia screening programs where available
