What is the Cholinergic System?
The cholinergic system refers to all neurons and neural pathways in the human body that use the neurotransmitter acetylcholine (ACh) to transmit signals. Acetylcholine was the first neurotransmitter ever discovered and plays a central role in both the central nervous system (CNS) and the peripheral nervous system (PNS). The cholinergic system influences a wide range of vital functions, including muscle movement, memory formation, attention, the sleep-wake cycle, and the regulation of internal organs.
Components of the Cholinergic System
Acetylcholine as a Neurotransmitter
Acetylcholine is synthesized in cholinergic neurons from the precursors choline and acetyl-CoA by the enzyme choline acetyltransferase (ChAT). After being released into the synaptic cleft, acetylcholine binds to specific receptors on the target cell and is then rapidly broken down by the enzyme acetylcholinesterase (AChE). The choline released in this process is taken back up by the presynaptic neuron and reused for acetylcholine synthesis.
Cholinergic Receptors
The effects of acetylcholine are mediated through two main types of receptors:
- Muscarinic receptors (M1-M5): These G-protein-coupled receptors are found mainly in glands, smooth muscle, the heart, and the brain. They regulate heart rate, glandular secretion, and smooth muscle tone, among other functions.
- Nicotinic receptors (nAChR): These ligand-gated ion channels are located at the neuromuscular junction (the connection between nerve and skeletal muscle), in autonomic ganglia, and in the brain. They mediate fast, excitatory signals.
Functions of the Cholinergic System
In the Peripheral Nervous System
In the peripheral nervous system, the cholinergic system is a key component of the parasympathetic nervous system (also known as the rest-and-digest system). It slows the heart rate, promotes digestion, stimulates glandular secretions, and causes constriction of the pupils (miosis). It also transmits signals at the neuromuscular junction, enabling voluntary skeletal muscle movement.
In the Central Nervous System
In the brain, the cholinergic system plays a critical role in:
- Learning and memory: Cholinergic neurons in the basal forebrain (particularly in the nucleus basalis of Meynert) project to the hippocampus and cortex and are essential for memory formation and cognitive function.
- Attention and wakefulness: The cholinergic system modulates the sleep-wake cycle and cortical arousal.
- Motor control: In the striatum, acetylcholine influences the balance between cholinergic and dopaminergic signals, which is necessary for coordinated movement.
Diseases Related to the Cholinergic System
Dysfunction of the cholinergic system is associated with various diseases:
- Alzheimer disease: This condition involves a significant loss of cholinergic neurons in the basal forebrain, leading to the characteristic symptoms of memory loss and cognitive decline. Many Alzheimer medications (e.g., cholinesterase inhibitors such as donepezil) target this system.
- Parkinson disease: An imbalance between acetylcholine and dopamine in the striatum contributes to the motor symptoms of this disease.
- Myasthenia gravis: An autoimmune disease in which antibodies are produced against nicotinic acetylcholine receptors at the neuromuscular junction, leading to muscle weakness.
- Organophosphate poisoning: Pesticides and certain nerve agents inhibit acetylcholinesterase, causing acetylcholine to accumulate and leading to overactivation of the cholinergic system (cholinergic crisis).
Pharmacological Interventions
Because the cholinergic system is involved in many diseases, a wide range of medications exist that specifically target this system:
- Cholinesterase inhibitors (e.g., donepezil, rivastigmine): Inhibit the breakdown of acetylcholine and are used to treat Alzheimer disease.
- Anticholinergics (e.g., atropine, ipratropium): Block muscarinic receptors and are used in the treatment of bradycardia, COPD, and overactive bladder.
- Muscle relaxants (e.g., succinylcholine): Act on nicotinic receptors and are used in anesthesia.
- Nicotinic agonists (e.g., nicotine, varenicline): Are used in smoking cessation therapy.
