What Is Chronic Pharyngitis?
Chronic pharyngitis is a persistent or recurrently occurring inflammation of the mucous membrane of the pharynx (throat). Unlike acute pharyngitis, which typically resolves within a few days, the chronic form lasts for weeks or months or keeps returning. It is a common condition that can affect people of all ages, particularly adults who are regularly exposed to irritants or suffer from underlying health conditions.
Causes
Chronic pharyngitis can be triggered or aggravated by a variety of factors:
- Persistent irritant exposure: Cigarette smoke, alcohol, dust, chemical fumes, and dry indoor air are among the most frequent causes.
- Chronic infections: Repeated bacterial or viral infections can cause lasting damage to the throat lining.
- Gastroesophageal reflux disease (GERD): Stomach acid rising into the throat is a frequently overlooked cause of chronic irritation.
- Allergies: Chronic allergic reactions lead to ongoing mucosal irritation.
- Mouth breathing: Obstruction of nasal airflow due to a deviated nasal septum or nasal polyps forces mouth breathing, which dries out the throat.
- Occupational stress: People who use their voice extensively (teachers, singers) or work in dusty environments are particularly at risk.
- Immune deficiency: A weakened immune system can increase susceptibility to recurring infections and inflammation.
Symptoms
The symptoms of chronic pharyngitis are generally less intense than those of acute infection but are more prolonged. Common symptoms include:
- Persistent sore throat or throat irritation
- Dry, irritating cough due to mucus accumulation
- Sensation of a foreign body in the throat (globus sensation)
- Difficulty swallowing (dysphagia)
- Hoarseness or voice changes
- Thick or sticky mucus in the throat
- Frequent throat clearing
- Bad breath (halitosis)
Types of Chronic Pharyngitis
Clinicians distinguish between several subtypes:
- Pharyngitis chronica simplex: Mild, diffuse redness and swelling of the throat mucosa without significant tissue changes.
- Pharyngitis chronica hyperplastica (granulosa): The mucosa is thickened and shows granular enlargements of lymphatic tissue on the posterior pharyngeal wall.
- Pharyngitis chronica atrophica (sicca): The mucosa becomes thin, dry, and shiny. This form is most common in older patients and causes a pronounced sensation of dryness.
Diagnosis
Diagnosis is typically made by an ear, nose, and throat (ENT) specialist. The evaluation includes:
- Throat examination: Visual inspection of the mucosa for redness, swelling, coatings, or tissue changes.
- Medical history: Assessment of risk factors such as smoking, occupation, reflux, or allergies.
- Throat swab: Taken when a bacterial infection is suspected.
- Endoscopy: Laryngoscopy or nasopharyngoscopy may be performed as needed.
- Allergy testing or reflux evaluation: Further investigations are initiated when these causes are suspected.
Treatment
Treatment of chronic pharyngitis is primarily directed at addressing the underlying cause. Identifying and eliminating triggers is therefore essential:
General Measures
- Smoking cessation and avoidance of alcohol
- Adequate fluid intake to keep mucous membranes moist
- Use of a humidifier indoors
- Regular gargling with saline or antiseptic solutions
- Avoidance of known irritants such as dust and chemical vapors
Medication
- Mucosal moisturizers: Throat sprays or lozenges to soothe and hydrate the pharynx.
- Antacids or proton pump inhibitors: Used when reflux is the underlying cause.
- Antihistamines: Helpful in allergy-related pharyngitis.
- Antibiotics: Only prescribed when a confirmed bacterial infection is present.
Treatment of Underlying Conditions
If a structural cause such as a deviated nasal septum, nasal polyps, or tonsillar hypertrophy is identified, surgical intervention may be recommended to restore nasal breathing and reduce stress on the throat lining.
Outlook and Prognosis
With consistent treatment of the underlying cause, chronic pharyngitis can be well managed and symptoms significantly reduced. However, if the triggering factors are not addressed, the condition tends to persist with ongoing discomfort. In rare cases, chronic mucosal inflammation may increase the risk of further complications. Regular medical follow-up is recommended.
