What is Co-trimoxazole?
Co-trimoxazole is a combination antibiotic composed of two active ingredients: trimethoprim and sulfamethoxazole in a fixed ratio of 1:5. The two substances work synergistically, meaning their combined effect is greater than the sum of their individual effects. Co-trimoxazole belongs to the sulfonamide class of antibiotics and has been used in medicine since the 1960s.
Mechanism of Action
Co-trimoxazole targets bacterial folic acid synthesis at two distinct steps:
- Sulfamethoxazole inhibits the enzyme dihydropteroate synthase, blocking an early step in bacterial folic acid production.
- Trimethoprim inhibits the enzyme dihydrofolate reductase, blocking a subsequent step in the same pathway.
Because bacteria – unlike human cells – must synthesize their own folic acid, this dual inhibition acts selectively against bacteria. Without folic acid, bacteria cannot produce DNA or essential proteins, ultimately leading to their death.
Indications
Co-trimoxazole is used to treat a wide range of bacterial infections, including:
- Urinary tract infections (including acute, uncomplicated cystitis)
- Respiratory tract infections (e.g., bronchitis, pneumonia caused by Pneumocystis jirovecii, especially in immunocompromised patients)
- Gastrointestinal infections (e.g., traveler's diarrhea caused by susceptible bacteria)
- Skin and soft tissue infections
- Toxoplasmosis and other opportunistic infections in HIV-positive patients
Dosage
The standard adult dose is 960 mg of co-trimoxazole (equivalent to 160 mg trimethoprim and 800 mg sulfamethoxazole) taken twice daily. Higher doses may be used for severe infections. Co-trimoxazole is available as tablets, oral suspension, and intravenous solution. The exact dose and duration of treatment should always be determined by a healthcare professional.
Side Effects
Like all medications, co-trimoxazole can cause side effects. The most common include:
- Nausea, vomiting, and diarrhea
- Skin rash and allergic reactions
- Blood count changes (e.g., anemia, reduced white blood cell count)
- Elevated kidney values (renal stress)
- Increased sensitivity to sunlight (photosensitivity)
In rare cases, serious skin reactions such as Stevens-Johnson syndrome may occur. At the first signs of severe skin changes, medical attention should be sought immediately.
Contraindications and Drug Interactions
Co-trimoxazole should not be used in patients with:
- Known allergy to sulfonamides or trimethoprim
- Severe liver or kidney disease
- Blood cell production disorders
- Newborns in the first four to six weeks of life
Important drug interactions exist with anticoagulants (blood thinners such as warfarin), methotrexate, diuretics, and certain antidiabetic medications. Concurrent use should be medically supervised.
Pregnancy and Breastfeeding
Co-trimoxazole should only be used during pregnancy – particularly in the first and third trimesters – and during breastfeeding after careful medical evaluation, as it may affect the folic acid metabolism of the infant.
Antibiotic Resistance
Due to widespread global use, some bacterial species have developed resistance to co-trimoxazole. Before initiating therapy, a susceptibility test (antibiogram) may be advisable to confirm effectiveness against the causative pathogen.
