Cytokine release refers to the secretion of cytokines by immune cells. It regulates inflammatory responses but can become dangerous when dysregulated, leading to a cytokine storm.
What Is Cytokine Release?
Cytokine release describes the process by which immune cells – particularly T cells, macrophages, and natural killer cells – secrete cytokines. Cytokines are small signaling proteins that coordinate communication between immune cells and regulate inflammatory responses. They play a central role in defending the body against infections, tumor cells, and other threats.
Biological Mechanism
When the immune system detects a pathogen or other threat, immune cells become activated and release various cytokines. The most important cytokines include:
- Interleukins (IL) – e.g., IL-1, IL-6, IL-2 – regulate the growth and differentiation of immune cells.
- Tumor necrosis factor-alpha (TNF-α) – promotes inflammatory responses and destruction of infected cells.
- Interferons (IFN) – inhibit viral replication and activate additional immune cells.
- Chemokines – attract further immune cells to sites of inflammation.
These cytokines act as messengers, coordinating the immune response by activating, inhibiting, or directing other cells to migrate to sites of inflammation.
Clinical Significance
Physiological Cytokine Release
Under normal circumstances, cytokine release is a tightly regulated and vital process. It enables a targeted and effective immune response against infections, injuries, and tumor cells. Once the threat has been successfully neutralized, cytokine secretion is downregulated through counter-regulatory mechanisms.
Pathological Cytokine Release – the Cytokine Storm
In some situations, cytokine release becomes dysregulated and leads to a so-called cytokine storm, also known as cytokine release syndrome (CRS). In this condition, excessive amounts of cytokines are released, triggering an uncontrolled systemic inflammatory response that can be life-threatening and lead to multi-organ failure.
A cytokine storm can be triggered by:
- Severe infections (e.g., influenza, COVID-19, sepsis)
- Immunotherapies, especially CAR T-cell therapies or bispecific antibodies
- Autoimmune diseases
- Transplant rejection reactions
Symptoms of Excessive Cytokine Release
Pathological cytokine release can manifest through the following symptoms:
- High fever and chills
- Severe fatigue and malaise
- Low blood pressure (hypotension)
- Rapid heart rate (tachycardia)
- Shortness of breath and pneumonia
- Organ failure (kidney, liver, lungs)
- Neurological symptoms such as confusion or seizures
Diagnosis
The diagnosis of excessive cytokine release is based on clinical findings and laboratory results. Important diagnostic markers include:
- Elevated levels of IL-6, TNF-α, and other cytokines in the blood
- Elevated C-reactive protein (CRP) and ferritin
- Elevated D-dimers indicating coagulation activation
- Imaging studies to assess affected organs
Treatment
Treatment of pathological cytokine release is tailored to the severity of the condition and includes:
- Corticosteroids (e.g., dexamethasone) to suppress the immune response
- IL-6 inhibitors such as tocilizumab or siltuximab, which specifically block the effects of interleukin-6
- JAK inhibitors (e.g., ruxolitinib) to inhibit cytokine signaling pathways
- Intensive care supportive therapy in cases of organ failure
- Specific therapies targeting the underlying cause
