Dabigatran: Uses, Mechanism and Side Effects

What is Dabigatran?
Dabigatran (as dabigatran etexilate) is an oral anticoagulant and a member of the direct thrombin inhibitor class of medications. It belongs to the group of novel oral anticoagulants (NOACs), also referred to as direct oral anticoagulants (DOACs), and is marketed under the brand name Pradaxa. Unlike older anticoagulants such as warfarin, dabigatran does not generally require regular blood monitoring, making it more convenient for many patients.
Mechanism of Action
Dabigatran etexilate is a prodrug that is converted to its active form, dabigatran, after absorption in the gut and liver. The active compound directly and reversibly inhibits thrombin (factor IIa), a key enzyme in the coagulation cascade. By blocking thrombin, dabigatran prevents the conversion of fibrinogen to fibrin, thereby inhibiting the formation of blood clots (thrombi).
Indications
- Prevention of stroke and systemic embolism in adult patients with non-valvular atrial fibrillation
- Treatment and prevention of deep vein thrombosis (DVT) and pulmonary embolism (PE)
- Primary prevention of venous thromboembolic events following elective hip or knee replacement surgery
Dosage
The standard dose for atrial fibrillation is 150 mg twice daily. A reduced dose of 110 mg twice daily is recommended for certain high-risk groups, such as patients over 80 years of age or those taking verapamil concurrently. Capsules should be swallowed whole with a full glass of water. Dose adjustment is required in patients with impaired kidney function, as dabigatran is primarily excreted via the kidneys.
Side Effects
As with all anticoagulants, the main risk associated with dabigatran is increased bleeding. Common and important side effects include:
- Bleeding (most common): nosebleeds, gum bleeding, blood in urine or stool, bruising
- Gastrointestinal complaints: nausea, heartburn, stomach pain or discomfort
- In rare cases: severe internal bleeding, intracranial hemorrhage
- Elevated liver enzymes (rare)
Contraindications and Precautions
Dabigatran must not be used in patients with:
- Severe renal impairment (creatinine clearance below 30 ml/min)
- Active, clinically significant bleeding
- Mechanical prosthetic heart valves
- Certain liver diseases associated with coagulopathy
- Concomitant use of certain medications (e.g., strong P-glycoprotein inhibitors such as ketoconazole or ciclosporin)
Patients should always inform their healthcare provider or dentist about dabigatran use before any surgical or dental procedure, as temporary discontinuation may be required.
Antidote
Since 2015, a specific reversal agent, idarucizumab (Praxbind), has been available to rapidly and completely neutralize the anticoagulant effect of dabigatran in life-threatening bleeding situations or emergency procedures. This represents a significant clinical advantage for patient safety.
