What Are Dennie-Morgan Lines?
Dennie-Morgan lines (also called Dennie-Morgan folds) are double or single fold lines that form on the lower eyelid just beneath the eyes. They are considered a classic clinical sign of atopic dermatitis (eczema) and are named after physicians Charles Clayton Dennie and William Perry Morgan, who described this finding.
These lines typically appear during childhood and may persist throughout a person's life. While they are not painful or medically dangerous, they serve as an important indicator of an atopic predisposition.
Causes and Development
The exact cause of Dennie-Morgan lines is not yet fully understood, but several contributing factors have been identified:
- Chronic inflammation: In atopic dermatitis, recurring inflammatory reactions of the skin can alter the connective tissue in the lower eyelid area.
- Allergic reactions: Frequent rubbing of the eyes due to itching or allergic rhinitis can contribute to fold formation.
- Genetic predisposition: Dennie-Morgan lines tend to run in families and are associated with an atopic constitution.
- Edema: Mild swelling in the lower eyelid area caused by allergic reactions can promote fold development.
Clinical Significance and Relation to Atopic Conditions
Dennie-Morgan lines are one of several minor criteria in the diagnostic criteria for atopic dermatitis according to Hanifin and Rajka. They frequently occur alongside other atopic conditions:
- Atopic dermatitis (eczema)
- Allergic rhinitis (hay fever)
- Allergic asthma
- Allergic conjunctivitis
The concurrent presence of these conditions is referred to as the atopic triad. Dennie-Morgan lines are especially common in children with atopic dermatitis but can also be observed in adults.
Diagnosis
The diagnosis of Dennie-Morgan lines is made purely on a clinical basis, meaning through visual inspection during a physical examination. No special tests or laboratory investigations are required to identify this sign.
As part of a broader evaluation for atopic disease, additional investigations may be useful:
- Allergy testing (skin prick test, specific IgE blood tests)
- Complete blood count with eosinophil count
- Total IgE level measurement
Treatment
Dennie-Morgan lines themselves do not require specific treatment, as they pose no medical risk. The focus is on treating the underlying atopic condition:
- Basic skin care: Regular use of emollient and moisturizing products to strengthen the skin barrier.
- Corticosteroids: Short-term application of topical corticosteroids during acute flare-ups of atopic dermatitis.
- Calcineurin inhibitors: Topical agents such as tacrolimus or pimecrolimus as steroid-free alternatives.
- Antihistamines: To relieve itching and allergic reactions.
- Allergen immunotherapy: In cases of confirmed allergies, specific immunotherapy (desensitization) may help reduce the severity of atopic reactions.
For cosmetic reasons, the folds can occasionally be addressed through dermatological or aesthetic procedures, although this is not medically necessary.
