Desloratadine is a second-generation antihistamine used to treat allergic conditions such as hay fever and chronic hives. It is non-sedating and generally well tolerated.
What is Desloratadine?
Desloratadine is an active pharmaceutical ingredient belonging to the second-generation antihistamine class. It is the primary active metabolite of loratadine and is widely used in the management of allergic conditions. Unlike older, first-generation antihistamines, desloratadine does not readily cross the blood-brain barrier, meaning it causes little to no drowsiness in most patients.
Indications
Desloratadine is indicated for the relief of symptoms associated with the following conditions:
- Allergic rhinitis (seasonal and perennial hay fever): including sneezing, nasal itching, runny nose, and watery eyes.
- Chronic idiopathic urticaria (hives): including skin welts, redness, and itching.
- Allergic reactions triggered by dust mites, pet dander, or mould spores.
Mechanism of Action
Desloratadine acts as a selective, peripheral histamine H1-receptor antagonist. It works by competitively blocking H1 receptors, preventing histamine -- a chemical messenger released during allergic reactions -- from binding to these receptors. This inhibition reduces the typical allergy symptoms such as itching, swelling, redness, and increased mucus secretion. Desloratadine also exhibits mild anti-inflammatory properties by suppressing the release of pro-inflammatory cytokines.
Dosage and Administration
The standard dose for adults and adolescents aged 12 years and older is 5 mg once daily, available as a tablet or orally disintegrating tablet. For younger children, age-appropriate doses in syrup form are recommended:
- Children aged 1 to 5 years: 1.25 mg per day
- Children aged 6 to 11 years: 2.5 mg per day
Desloratadine can be taken with or without food. Dose adjustment may be necessary in patients with severe renal or hepatic impairment.
Side Effects
Desloratadine is considered to have a favorable safety profile. Commonly reported side effects include:
Serious adverse reactions such as anaphylaxis or cardiac arrhythmias are very rare but possible. The medication should not be used in patients with known hypersensitivity to desloratadine or loratadine.
Interactions and Contraindications
Desloratadine has relatively few clinically relevant drug interactions compared to older antihistamines. Concurrent alcohol consumption may slightly enhance sedative effects. Dose adjustments should be considered in patients with severe renal impairment. The use of desloratadine during pregnancy and breastfeeding should only be considered following medical advice.
