What is Dimenhydrinate?
Dimenhydrinate is a first-generation antihistamine medication primarily used to prevent and treat nausea, vomiting, and dizziness caused by motion sickness (kinetosis). It is widely known under brand names such as Dramamine or Gravol and is available over the counter in pharmacies worldwide.
Mechanism of Action
Dimenhydrinate is a combination of diphenhydramine (the active compound) and 8-chlorotheophylline. Its therapeutic effects result from the blockade of histamine H1 receptors and muscarinic acetylcholine receptors in the central nervous system.
- It inhibits the transmission of balance signals from the vestibular apparatus (inner ear) to the vomiting center in the brain.
- By blocking the vomiting center and the vomiting reflex arc, it suppresses nausea and vomiting.
- Its sedative effect arises from H1 receptor blockade in the brain.
Indications
Dimenhydrinate is used for the following conditions:
- Motion sickness: nausea and vomiting during travel by car, ship, airplane, or train
- Dizziness and balance disorders (e.g., in Meniere disease)
- Postoperative nausea (under medical supervision)
- Nausea and vomiting of various origins
Dosage and Administration
The recommended adult dose is generally 50 to 100 mg per intake, with a maximum of 300 to 400 mg per day. For motion sickness prevention, the first dose should be taken approximately 30 minutes before departure. Pediatric dosing must be carefully adjusted based on weight and age.
Available formulations include tablets, chewing gum, suppositories, and injectable solutions.
Side Effects
Common side effects of dimenhydrinate include:
- Sedation and drowsiness (very common)
- Dry mouth
- Blurred vision
- Urinary retention
- Impaired concentration and coordination
- Rapid heartbeat (tachycardia)
Due to its sedative effects, patients should not drive or operate machinery after taking dimenhydrinate.
Contraindications and Drug Interactions
Dimenhydrinate should not be used in cases of:
- Narrow-angle glaucoma
- Benign prostatic hyperplasia with urinary retention
- Epilepsy
- Pheochromocytoma
- Pregnancy (especially the first trimester) and breastfeeding without medical advice
Important drug interactions occur with:
- Alcohol and other central nervous system depressants (enhanced sedation)
- MAO inhibitors (dangerous interactions)
- Anticholinergic drugs (worsening of side effects such as dry mouth and urinary retention)
Special Warnings
Dimenhydrinate carries a risk of misuse at high doses, as very large amounts can induce hallucinogenic effects. For this reason, its sale is subject to pharmacy restrictions in several countries. Long-term use without medical supervision is not recommended.
