What is Double Vision Therapy?
Double vision therapy refers to a range of therapeutic approaches used to treat diplopia -- the medical term for double vision. People with diplopia perceive a single object as two separate images because their eyes are not working together properly. The goal of therapy is to address the underlying cause of double vision or to reduce its impact, thereby improving the quality of life of those affected.
Causes of Double Vision
Double vision occurs when the alignment of the two eyes is disrupted and the brain cannot merge the two images into one. Common causes include:
- Extraocular muscle palsies (e.g., caused by stroke, diabetes, or cranial nerve lesions)
- Strabismus (squint) that develops in adulthood or remains untreated
- Orbital diseases (e.g., thyroid eye disease, also known as Graves orbitopathy)
- Trauma to the skull or orbit
- Neurological conditions such as multiple sclerosis or myasthenia gravis
- Brain tumors or raised intracranial pressure
Diagnosis
Before starting double vision therapy, a thorough ophthalmological and neurological evaluation is performed. Diagnostic methods include:
- Cover test and uncover test to assess eye alignment
- Hess screen test to precisely map eye movement disorders
- Imaging studies such as MRI or CT to rule out neurological causes
- Blood tests when a systemic condition is suspected
Treatment Methods
Prism Correction
One of the most important conservative treatments is the use of prism lenses. Prisms redirect incoming light so that the images from both eyes are realigned. This can be provided as a Fresnel (stick-on foil) prism or as a prism ground directly into spectacle lenses. Prism correction offers quick relief but does not treat the underlying cause.
Orthoptic Exercise Therapy
Orthoptics is a specialty within ophthalmology that focuses on disorders of eye movement and binocular vision. Orthoptists guide patients through targeted eye muscle exercises designed to improve the coordination of both eyes. These include:
- Fixation exercises to strengthen ocular motor control
- Fusion exercises to improve the merging of images from both eyes
- Stereoscopic exercises to develop depth perception
Occlusion (Patching One Eye)
In cases of acute or temporary double vision, covering one eye with an occlusion patch or foil can provide immediate relief by suppressing the second image. However, this approach does not promote binocular vision and is therefore generally used only as a short-term measure.
Botulinum Toxin Injections
For certain types of eye muscle misalignment, injections of botulinum toxin into the affected muscle can temporarily weaken it and correct eye positioning. This method is used especially when surgery is not yet appropriate or as preparation for a surgical procedure.
Surgical Treatment
When conservative measures are insufficient, surgery on the extraocular muscles (strabismus surgery) may be required. The procedure involves shortening, lengthening, or repositioning the eye muscles to achieve a permanent correction of eye alignment. Surgery is usually performed under general anesthesia and is a routine procedure in ophthalmology.
Treatment of the Underlying Condition
When double vision is caused by an underlying disease (e.g., diabetes, thyroid disease, myasthenia gravis), treating that root cause is the primary focus. In many cases, double vision improves once the underlying condition is successfully managed.
Prognosis and Outlook
The outcome of double vision therapy depends greatly on the underlying cause. Many cases of diplopia resulting from stroke or cranial nerve palsy improve spontaneously within weeks to months. For permanent misalignments, surgical or optical treatment can in most cases achieve significant improvement or full resolution of symptoms. Regular follow-up with an ophthalmologist or orthoptist is important to monitor progress.
