Initial medical care encompasses all immediate actions taken following an accident, sudden illness, or emergency situation to preserve a patient´s life and stabilize their condition until professional help arrives. It represents the first link in the chain of medical care and can be performed by both trained medical personnel and laypersons.
Initial medical care is a central concept in emergency medicine and rescue services. It includes basic first aid measures as well as professional pre-hospital care provided by paramedics and emergency physicians.
Goals of Initial Medical Care
The primary objective is to maintain or restore the patient´s vital functions. Specifically, this includes:
- Securing the airway and supporting breathing
- Maintaining cardiovascular function, including chest compressions if necessary
- Controlling severe bleeding
- Preventing shock and further complications
- Protection against hypothermia (dangerous loss of body heat)
- Providing psychological support and reassurance to the patient
Measures in Initial Medical Care
Layperson Measures (First Aid)
Even without medical training, important life-saving steps can be taken. These include calling the emergency services (in Germany: 112), placing an unconscious but breathing person in the recovery position, performing cardiopulmonary resuscitation (CPR) in the event of cardiac arrest, and applying pressure bandages to control bleeding.
Professional Pre-Hospital Care
Emergency medical technicians, paramedics, and emergency physicians carry out advanced procedures at the scene, including:
- Advanced airway management (e.g., intubation, laryngeal mask airway)
- Intravenous access and fluid therapy
- Administration of emergency medications
- ECG monitoring and defibrillation for ventricular fibrillation
- Immobilization for suspected spinal or fracture injuries
- Pain management
Initial Care in the Hospital
In the emergency department, initial care is continued by an interdisciplinary team. The trauma bay (or resuscitation room) is used for critically injured or ill patients. Structured protocols such as the ABCDE approach (Airway, Breathing, Circulation, Disability, Exposure) are applied to systematically identify and treat all life-threatening conditions.
Initial Care in Specific Emergency Situations
Cardiac Arrest
Every second counts during cardiac arrest. Immediate initiation of chest compressions and, when available, the use of an automated external defibrillator (AED) significantly increases survival rates.
Severe Bleeding
In cases of life-threatening bleeding, pressure bandages are applied or, for limb injuries, a tourniquet is used to minimize blood loss.
Loss of Consciousness
An unconscious person who is still breathing is placed in the recovery position to prevent the airway from becoming blocked by vomit or the tongue.
Burns and Scalds
Affected areas are immediately cooled with cool (not ice-cold) water and covered with a sterile dressing. Extensive burns require immediate professional medical attention.
The Importance of First Aid Knowledge
Studies show that early intervention by laypersons can significantly reduce mortality in cardiac arrest and other emergencies. Regular refresher courses are recommended by organizations such as the European Resuscitation Council (ERC) and the Red Cross to maintain competency in first aid.
