What is Functional Diagnostics?
Functional diagnostics is a specialised examination method used in dentistry and orthodontics to analyse the function of the entire masticatory system. This includes the temporomandibular joints (TMJ), the chewing muscles, the teeth, and their interaction during biting, chewing, and speaking. The primary goal is the early detection and targeted treatment of functional disorders and misalignments.
When is Functional Diagnostics Used?
Functional diagnostics is indicated when patients experience symptoms that may point to a craniomandibular dysfunction (CMD) or other disorders of the masticatory system. Common reasons include:
- Pain in the jaw joint or chewing muscles
- Headaches and neck tension
- Clicking or grinding sounds in the jaw joint
- Teeth grinding (bruxism) or clenching
- Restricted mouth opening
- Pain during chewing or yawning
- Prior to extensive prosthetic or orthodontic treatment
The Functional Diagnostics Process
Clinical Functional Analysis
The clinical functional analysis is the first step and is carried out directly on the patient. The dentist manually examines the temporomandibular joints, palpates the chewing muscles, and assesses the range of motion of the lower jaw. Joint sounds and pain responses to pressure are systematically documented.
Instrumental Functional Analysis
For more complex findings, an instrumental functional analysis is performed. Dental models are mounted in a device called an articulator, which mechanically simulates jaw movements. A face bow is used to transfer the individual position of the upper jaw in relation to the skull. This allows precise analysis of tooth contacts, bite height, and jaw movements.
Imaging Procedures
Imaging techniques may be used as a supplement to visualise the structures of the jaw joint. These include:
- Panoramic X-ray (OPG): An overview image of the teeth and jaw joints
- MRI (Magnetic Resonance Imaging): Detailed visualisation of cartilage and soft tissue in the jaw joint
- CT (Computed Tomography): Imaging of bony structures
Craniomandibular Dysfunction (CMD)
The most common indication for functional diagnostics is craniomandibular dysfunction (CMD). This is a group of conditions affecting the temporomandibular joints, chewing muscles, and surrounding structures. CMD can lead to chronic pain, sleep disturbances, and a significant reduction in quality of life. Functional diagnostics enables a precise diagnosis and forms the basis for targeted therapy.
Treatment Options Following Diagnosis
Based on the results of functional diagnostics, various treatment approaches can be initiated:
- Occlusal splint: A custom-fitted bite guard that relieves pressure on the jaw joints and reduces bruxism
- Physiotherapy: Targeted exercises to relax and strengthen the chewing muscles
- Occlusal adjustment: Correction of disruptive tooth contacts
- Prosthetic or orthodontic measures: For structural causes
- Relaxation therapy and biofeedback: For psychosomatic components
The Importance of Functional Diagnostics
Early functional diagnostics can prevent secondary damage to the teeth, jaw joints, and surrounding structures. It is also essential before planning dental prosthetics, implants, or orthodontic treatments, as undetected functional disorders can compromise treatment outcomes.
