The heel walking test primarily assesses the function of the common peroneal nerve (also called the fibular nerve) and the anterior compartment muscles of the lower leg, particularly the foot dorsiflexors. It also provides information about a patient's coordination and balance.

- Lumbar disc herniation: Particularly at levels L4/L5, which can affect nerve roots controlling foot dorsiflexion
- Polyneuropathy: Widespread peripheral nerve damage, as seen in diabetic neuropathy
- Spinal cord disorders, such as multiple sclerosis
Orthopedic and Muscular Causes
- Weakness or atrophy of the anterior lower leg muscles
- Injuries or inflammation around the ankle joint
- Pain conditions that make heel weight-bearing difficult
