These conditions primarily affect the cerebellum, cerebellar pathways, and the spinal cord, resulting in gradually worsening motor dysfunction.
Hereditary ataxias are classified as rare diseases and can manifest at any age, although many forms begin in childhood or early adulthood. The most clinically recognized subgroups include the spinocerebellar ataxias (SCA) and Friedreich ataxia.
Causes and Genetic Basis
Hereditary ataxias are caused by mutations in specific genes essential for the survival and function of neurons. The clinical presentation varies considerably depending on the gene affected and the mode of inheritance.
- Autosomal dominant hereditary ataxias: This group includes the spinocerebellar ataxias (SCA types 1-48+), where a single copy of the mutated gene is sufficient to cause disease. Many are caused by trinucleotide repeat expansions.
- Autosomal recessive hereditary ataxias: Friedreich ataxia is the most common form in this category. Both copies of the gene must be affected. It is most often caused by a GAA repeat expansion in the FXN gene, which encodes the protein frataxin.
- X-linked forms: Less common examples include the Fragile X-associated Tremor/Ataxia Syndrome (FXTAS).
- Mitochondrial ataxias: Caused by mutations in mitochondrial DNA that impair energy metabolism in nerve cells.
Symptoms
Symptoms typically develop gradually and worsen over years to decades. Common manifestations include:
- Gait disturbances: Unsteady, wide-based walking with staggering (cerebellar gait)
- Limb incoordination: Difficulty with grasping, writing, or fine motor tasks (dysmetria)
- Speech difficulties: Slurred or scanning speech (dysarthria)
- Eye movement abnormalities: Involuntary eye movements (nystagmus), impaired smooth pursuit
- Muscle weakness and spasticity (depending on subtype)
- Sensory disturbances: Numbness or tingling in the hands and feet
- Cardiac involvement: Especially in Friedreich ataxia (hypertrophic cardiomyopathy)
- Cognitive impairment may develop over time in some forms
Diagnosis
Diagnosing hereditary ataxia requires a thorough clinical evaluation combined with several diagnostic investigations.
Clinical Examination
A neurologist assesses gait, coordination, reflexes, and eye movements. Standard tests include the finger-nose test and the heel-shin test to evaluate coordination deficits.
Neuroimaging
Magnetic resonance imaging (MRI) of the brain and spinal cord frequently reveals atrophy of the cerebellum or brainstem, providing important clues about the subtype.
Genetic Testing
Molecular genetic analysis is the gold standard for definitive diagnosis. Targeted gene sequencing or multi-gene ataxia panels can identify the specific mutation responsible. Genetic counseling is strongly recommended for patients and their families.

- Electrophysiology: Nerve conduction studies (NCS) and evoked potentials to assess nerve function
- Echocardiography: Cardiac evaluation, especially important in Friedreich ataxia
- Laboratory tests: To exclude acquired causes such as vitamin E deficiency or gluten ataxia
Treatment
For most forms of hereditary ataxia, no disease-modifying cure currently exists. Management focuses on symptom relief and maintaining quality of life.
Pharmacological Therapy
- For Friedreich ataxia, omaveloxolone (Skyclarys) received FDA approval in 2023 as the first disease-modifying therapy. It activates the Nrf2 pathway, improving mitochondrial function and reducing oxidative stress.
- Symptomatic medications may address spasticity (e.g., baclofen), tremor, or cardiac complications.
Physiotherapy and Exercise
Physiotherapy is a cornerstone of management. Targeted balance and coordination training helps preserve motor function and reduces the risk of falls.
Speech and Language Therapy
Speech therapy addresses dysarthria and dysphagia, supporting effective communication and safe swallowing.
