What is Sodium Picosulfate?
Sodium picosulfate is a well-established active ingredient belonging to the class of laxatives. It is primarily used for the short-term treatment of constipation and for bowel cleansing prior to diagnostic or surgical procedures, such as a colonoscopy. The medication is available without a prescription in many countries and is one of the most widely used laxatives worldwide.
Indications
Sodium picosulfate is indicated for:
- Acute and chronic constipation
- Bowel preparation before colonoscopy, rectoscopy, or intestinal surgery
- Supporting bowel movements in bedridden patients
- Prevention of straining in patients with hemorrhoids or following rectal procedures
Mechanism of Action
Sodium picosulfate belongs to the class of stimulant laxatives. After oral administration, the active substance is converted by intestinal bacteria in the colon into its pharmacologically active form. This active metabolite directly stimulates the mucous membrane of the large intestine, increasing bowel motility (peristalsis). At the same time, sodium picosulfate inhibits the reabsorption of water and electrolytes from the intestinal wall, resulting in softer stools that are easier to pass.
Since the conversion to the active form takes place in the colon, the laxative effect typically begins after 6 to 12 hours. When taken in the evening, a bowel movement can therefore be expected the following morning.
Dosage and Administration
Sodium picosulfate is available as drops, tablets, or in combination with other substances as a powder for oral solution. The standard dosage for adults is:
- Drops: 10-18 drops (approximately 5-9 mg) once daily, preferably in the evening
- Tablets: 1 tablet (5 mg) once daily, in the evening
Reduced doses apply for children and should be determined by a healthcare professional. The medication should not be used for longer than 1-2 weeks without medical advice.
Side Effects
Like all medications, sodium picosulfate can cause side effects. Commonly reported side effects include:
- Abdominal cramps and bloating
- Diarrhea with excessive doses
- Nausea
With prolonged use or overdose, the following may occur:
- Electrolyte imbalances, particularly low potassium levels (hypokalemia)
- Laxative dependence
- Weakening of the intestinal muscles
Contraindications and Warnings
Sodium picosulfate should not be used in cases of:
- Bowel obstruction (ileus) or suspected ileus
- Acute inflammatory conditions of the gastrointestinal tract (e.g., appendicitis, Crohn's disease, acute ulcerative colitis)
- Severe dehydration
- Severe kidney impairment
During pregnancy and breastfeeding, sodium picosulfate should only be used after consultation with a healthcare provider, as the substance may pass into breast milk in small amounts.
Drug Interactions
Relevant interactions include:
- Diuretics and corticosteroids: Increased risk of electrolyte disturbances
- Antibiotics: May alter gut flora and reduce the conversion of sodium picosulfate into its active form
- Cardiac glycosides (e.g., digoxin): Hypokalemia caused by sodium picosulfate may enhance the effects of cardiac glycosides
