What is Salbutamol?
Salbutamol (known as albuterol in the United States) is a short-acting beta-2 agonist (SABA) widely used in the management of asthma and chronic obstructive pulmonary disease (COPD). It is one of the most commonly prescribed respiratory medications globally and is used as a reliever or rescue inhaler during acute episodes of breathlessness or wheezing.
Mechanism of Action
Salbutamol acts by selectively binding to beta-2 adrenergic receptors located in the smooth muscle of the bronchi. This binding stimulates the enzyme adenylyl cyclase, leading to an increase in intracellular cyclic adenosine monophosphate (cAMP). Elevated cAMP levels cause relaxation of the bronchial smooth muscle, resulting in bronchodilation -- a widening of the airways. The onset of action typically occurs within 3 to 5 minutes, with effects lasting 4 to 6 hours.
Indications
- Asthma bronchiale: Relief and prevention of acute bronchospasm
- COPD: Symptomatic relief of breathlessness and airflow obstruction
- Exercise-induced bronchoconstriction: Prevention when inhaled prior to physical activity
- Hyperkalaemia: Used clinically to rapidly lower elevated blood potassium levels
Dosage Forms and Dosing
Salbutamol is available in several formulations:
- Metered-dose inhaler (MDI / puffer): The most common form; the usual dose for acute relief is 100-200 micrograms (1-2 puffs)
- Dry powder inhaler: An alternative to liquid inhalation
- Nebuliser solution: Used in severe attacks or for young children unable to use an inhaler
- Tablets and syrup: Rarely used, as inhalation provides faster and more targeted delivery
- Injection solution: Reserved for severe bronchospasm in clinical settings
The appropriate dose depends on the patient's age, the severity of the condition, and the formulation used. Always follow the prescribing instructions provided by a healthcare professional.
Side Effects
Salbutamol is generally well tolerated at therapeutic doses, but the following side effects may occur:
- Common: Tremor, palpitations, tachycardia, headache, nervousness
- Occasional: Muscle cramps, mild decrease in blood pressure, insomnia
- Rare: Paradoxical bronchospasm (airway narrowing following inhalation), hypokalaemia (low blood potassium) at high doses
Paradoxical bronchospasm is a serious adverse reaction and requires immediate medical attention.
Contraindications and Interactions
Salbutamol should be used with caution in:
- Patients with serious cardiac conditions such as arrhythmias
- Patients with hyperthyroidism (overactive thyroid)
- Patients taking non-selective beta-blockers, which can reduce the effectiveness of salbutamol
Concurrent use with other sympathomimetic agents may increase the risk of cardiovascular side effects.
Important Notes on Use
Salbutamol is a reliever medication intended for use as needed -- not as a regular daily treatment. Frequent use (more than 2 days per week) may indicate poorly controlled asthma and should prompt a review by a healthcare provider. It does not replace anti-inflammatory maintenance therapy, such as inhaled corticosteroids, which address the underlying airway inflammation in asthma.
