What is Tamsulosin?
Tamsulosin (also known by its Latin name Tamsulosinum) is a prescription medication belonging to the class of alpha-1 adrenoceptor blockers (alpha-blockers). It is primarily used to relieve lower urinary tract symptoms (LUTS) associated with benign prostatic hyperplasia (BPH) – a non-cancerous enlargement of the prostate gland that commonly affects men as they age.
Indications
Tamsulosin is indicated for:
- Difficulty starting or maintaining urine flow due to BPH
- Frequent urination, including at night (nocturia)
- Weak or intermittent urinary stream
- Feeling of incomplete bladder emptying
In some countries, it is also used off-label to facilitate the spontaneous passage of small kidney or ureteral stones (medical expulsive therapy).
Mechanism of Action
Tamsulosin selectively blocks alpha-1A and alpha-1D adrenoceptors located in the smooth muscle of the prostate, bladder neck, and urethra. By blocking these receptors, it causes relaxation of the smooth muscle tissue, reducing the pressure on the urethra and improving urine flow. Compared to non-selective alpha-blockers, tamsulosin has minimal effects on blood vessels, making significant drops in blood pressure less likely.
Dosage and Administration
The standard dose is 0.4 mg once daily, preferably taken after breakfast. Tablets or capsules should be swallowed whole and must not be chewed, crushed, or opened. The duration of treatment is determined by the prescribing physician based on clinical response.
Side Effects
Tamsulosin is generally well tolerated. Possible side effects include:
- Common: Dizziness, ejaculation disorders (retrograde ejaculation)
- Uncommon: Headache, palpitations, orthostatic hypotension (drop in blood pressure upon standing)
- Rare: Allergic reactions, nosebleeds, blurred vision
- Important note: Patients scheduled for cataract surgery should inform their eye surgeon about tamsulosin use, as it is associated with Intraoperative Floppy Iris Syndrome (IFIS), which can complicate the procedure.
Drug Interactions
Tamsulosin may interact with the following substances:
- Other alpha-blockers or antihypertensives: Enhanced blood pressure-lowering effect
- PDE-5 inhibitors (e.g., sildenafil): Increased risk of hypotension
- Strong CYP3A4 inhibitors (e.g., ketoconazole): Possible increase in tamsulosin plasma levels
Contraindications
- Known hypersensitivity to tamsulosin
- Severe hepatic impairment
- History of orthostatic hypotension
