What Is Tear Fluid?
Tear fluid is a complex aqueous secretion continuously produced by the lacrimal glands and accessory glands of the conjunctiva and eyelids. It coats the surface of the eye as a thin protective layer known as the tear film, which is essential for the health, function, and comfort of the eye.
Composition
Tear fluid consists of three distinct overlapping layers that together form the tear film:
- Lipid layer (outer layer): Produced by the Meibomian glands of the eyelids, this layer reduces evaporation of the underlying aqueous layer.
- Aqueous layer (middle layer): The thickest layer, secreted by the main lacrimal gland and accessory lacrimal glands. It contains water, electrolytes, proteins, antibodies, and growth factors.
- Mucin layer (inner layer): Produced by goblet cells of the conjunctiva, this layer ensures the even spread of tear fluid across the ocular surface.
Key components of tear fluid include:
- Water (more than 98%)
- Electrolytes such as sodium, potassium, calcium, and chloride
- Proteins including lysozyme, lactoferrin, and immunoglobulin A (IgA)
- Lipids and mucins
- Trace amounts of glucose and urea
- Growth factors and cytokines
Functions
Tear fluid performs several vital functions in and around the eye:
- Lubrication: It keeps the cornea and conjunctiva moist, preventing dryness and discomfort.
- Protection: Antimicrobial proteins such as lysozyme and lactoferrin defend the eye against bacteria, viruses, and other pathogens.
- Nutrition: The avascular cornea (which lacks blood vessels) relies on tear fluid for oxygen and nutrient supply.
- Optical clarity: A stable, smooth tear film provides the primary refractive surface of the eye, which is essential for sharp vision.
- Cleansing: Tear fluid washes away foreign particles, dust, and cellular debris from the ocular surface.
- Emotional and reflex response: Tear production increases reflexively in response to emotional stimuli or physical irritants such as smoke or foreign bodies.
Production and Drainage
Tear fluid is continuously secreted by the main lacrimal gland and accessory lacrimal glands (glands of Krause and Wolfring). During blinking, the fluid is spread evenly across the eye surface. Drainage occurs through the lacrimal puncta at the inner corners of the eyelids, into the nasolacrimal duct, and ultimately into the nasal cavity -- which explains why crying is often accompanied by a runny nose.
Clinical Relevance
Disruptions in the quantity or quality of tear fluid can lead to a range of ocular conditions:
- Dry eye disease (keratoconjunctivitis sicca): Insufficient production or poor quality of the tear film causes burning, irritation, and blurred vision.
- Epiphora (watery eyes): Excessive tear production or impaired drainage results in persistently watering eyes.
- Allergic conjunctivitis: Allergic reactions alter the composition of tear fluid, triggering inflammation of the conjunctiva.
- Sjogren syndrome: An autoimmune condition in which the lacrimal and salivary glands are targeted, leading to severe dry eye.
Analysis of tear fluid is increasingly used as a diagnostic tool, as it may contain biomarkers for systemic diseases such as diabetes mellitus and neurological conditions.
