What Is Tenderness?
Tenderness (also called point tenderness or pressure tenderness) describes pain or sensitivity that is elicited by applying pressure to a specific area of the body. It is not a disease in itself but rather a clinical sign identified during physical examination. The affected area reacts painfully to touch or pressure, even though the person may experience little or no pain at rest.
Causes
Tenderness can occur in almost any part of the body and has a wide range of causes. Common triggers include:
- Inflammation: such as appendicitis, tendon sheath inflammation (tenosynovitis), or bursitis
- Injuries: bruising, muscle strains, torn muscle fibers, or bone fractures
- Infections: localized infections such as abscesses or enlarged lymph nodes
- Internal organ conditions: e.g., tenderness in the upper abdomen due to gallbladder inflammation or a stomach ulcer
- Musculoskeletal conditions: such as fibromyalgia, arthritis, or tendinopathy
- Vascular conditions: such as thrombophlebitis (inflammation of a vein)
Common Locations and Their Significance
Abdomen
Abdominal tenderness is an important diagnostic sign. Tenderness in the lower right abdomen may indicate appendicitis. Pain in the upper right abdomen is typical of gallbladder or liver disease, while tenderness in the lower left abdomen may suggest diverticulitis (inflammation of pouches in the intestinal wall).
Joints and Muscles
Tenderness around joints is commonly found in arthritis, gout, or following trauma. Muscles may become tender due to overuse, inflammation, or injury. In fibromyalgia, characteristic tender points occur at specific locations throughout the body.
Bone
Bone tenderness may point to a fracture, bone infection (osteomyelitis), or, in rare cases, a bone tumor.
Lymph Nodes
Tender, swollen lymph nodes are often a sign of a nearby infection or inflammatory process.
Diagnosis
Tenderness is assessed during a clinical physical examination through systematic palpation -- the methodical pressing and feeling of body regions. The clinician evaluates:
- the exact location of the tenderness
- the intensity of the pain response
- whether the pain radiates to other areas
- accompanying signs such as swelling, redness, or warmth
Depending on the suspected cause, further diagnostic measures may be ordered, including blood tests, ultrasound, X-ray, or MRI.
Treatment
Treatment of tenderness always targets the underlying cause. General measures may include:
- Rest and protection of the affected area
- Ice or cooling for acute injuries or inflammation
- Pain-relieving medication (e.g., ibuprofen, paracetamol)
- Physiotherapy for musculoskeletal causes
- Antibiotics for bacterial infections
- Surgery for serious conditions (e.g., appendicitis, abscess drainage)
When to See a Doctor
Tenderness should be evaluated by a medical professional if it:
- comes on suddenly and is severe
- is accompanied by fever, swelling, or redness
- persists for several days or gets worse over time
