What Are the Tonsils?
The tonsils are masses of lymphatic tissue situated in the throat and oral cavity. They are part of a ring of lymphoid tissue known as Waldeyer's tonsillar ring and play a key role in the body's immune defence. Their primary function is to detect and respond to pathogens such as bacteria and viruses that enter the body through the mouth and nose.
Types of Tonsils
There are several types of tonsils in the human body:
- Palatine tonsils: The most well-known tonsils, visible on both sides of the back of the throat.
- Pharyngeal tonsil (adenoids): Located in the upper part of the throat behind the nose; often enlarged in children.
- Lingual tonsil: Located at the base of the tongue.
- Tubal tonsils: Found near the opening of the Eustachian tubes.
Function of the Tonsils
The tonsils contain large numbers of immune cells called lymphocytes, which identify and fight incoming pathogens. They are particularly active during childhood, when the immune system is still developing. In adults, the tonsils play a less prominent role but remain part of the immune defence network.
Common Tonsil Conditions
Tonsillitis
Tonsillitis is an inflammation of the palatine tonsils, most commonly caused by bacterial infections (particularly group A Streptococcus) or viruses. Symptoms include sore throat, difficulty swallowing, fever, red and swollen tonsils, and white or yellowish patches on the tonsils. Bacterial tonsillitis is typically treated with antibiotics.
Chronic Tonsillitis
Chronic tonsillitis refers to persistent or recurrent inflammation of the tonsils. It can cause ongoing throat discomfort, bad breath, fatigue, and general malaise. Surgical removal of the tonsils (tonsillectomy) is often considered in these cases.
Peritonsillar Abscess
A peritonsillar abscess is a serious complication of tonsillitis, involving a collection of pus behind or beside the tonsil. It requires prompt medical attention, typically involving drainage of the abscess and antibiotic therapy.
Enlarged Adenoids
In children, the pharyngeal tonsil (adenoids) can become significantly enlarged, causing nasal obstruction, snoring, sleep disturbances, and recurrent ear infections. Surgical removal (adenoidectomy) may be recommended in severe cases.
Diagnosis
Tonsil conditions are typically diagnosed through:
- Physical examination of the throat
- Throat swabs to identify the causative organism
- Blood tests (e.g., full blood count, CRP levels for bacterial infection)
- Imaging (e.g., ultrasound or MRI if an abscess or tumour is suspected)
Treatment
Treatment depends on the type and severity of the condition:
- Conservative treatment: Antibiotics for bacterial infections, pain relief, rest, and adequate fluid intake.
- Surgical treatment: For recurrent or chronic tonsillitis, a tonsillectomy (complete removal) or tonsillotomy (partial reduction) may be performed.
