What is a Drainage Position?
A drainage position refers to a specific placement of the body or individual body parts designed to facilitate the outflow of fluids, secretions, or pus from body cavities, wounds, or the lungs using the force of gravity. It is used in clinical medicine, physiotherapy, and nursing care.
Areas of Application
Drainage positioning is applied across a range of medical contexts:
- Respiratory conditions: In diseases such as chronic bronchitis, bronchiectasis, or cystic fibrosis, postural drainage is used to clear mucus from specific lung segments by placing the patient in targeted positions.
- Wound care and surgery: After operations or in cases of infected wounds, patients are positioned to allow optimal drainage of wound secretions or pus.
- Pleural effusion: When fluid accumulates in the chest cavity, drainage positioning can support fluid removal through an inserted drain.
- Abscesses and infections: Drainage positions help empty collections of pus from body cavities using gravity.
Postural Drainage of the Airways
Postural drainage is one of the most common applications of the drainage position. The upper body is placed so that the affected lung segment is positioned uppermost, allowing secretions to flow toward the central airways, where they can be coughed up. Depending on the affected lung segment, different positions may be used, including:
- Head-down tilt (Trendelenburg position): The head is lower than the torso, promoting drainage of the lower lobes.
- Lateral (side-lying) position: The patient lies on the healthy side to drain the affected side.
- Prone position: Used to drain certain posterior lung segments.
Technique and Accompanying Measures
Drainage positions are frequently combined with additional therapeutic measures to enhance their effectiveness:
- Percussion (chest clapping): Rhythmic clapping on the chest wall to loosen retained secretions.
- Vibration: Vibrating hand movements along the chest to mobilize secretions.
- Breathing exercises: Targeted breathing techniques to support secretion clearance.
- Inhalation therapy: Inhalation of humidified air or mucolytic agents prior to drainage positioning.
Contraindications
Drainage positioning is not suitable for all patients. Key contraindications include:
- Severe heart failure or cardiac arrhythmias
- Recent myocardial infarction or unstable angina
- Raised intracranial pressure (contraindication for head-down tilt)
- Severe osteoporosis or fractures
- Active bleeding or pulmonary embolism
- Certain postoperative restrictions
Relevance in Nursing and Physiotherapy
In nursing and physiotherapy practice, the correct instruction and application of drainage positions is an essential skill. Practitioners pay attention to the individual tolerance of the patient, the duration of each position (typically 10 to 20 minutes), and the hygienic management of any equipment used.
