What is Erythropoietin?
Erythropoietin (abbreviated EPO) is a glycoprotein hormone produced primarily by the kidneys. It is the main growth factor responsible for stimulating the production of red blood cells (erythrocytes) in the bone marrow. Small amounts of EPO are also produced in the liver.
Mechanism of Action
Erythropoietin is released in response to low oxygen levels in the blood – a condition known as hypoxia. This can occur during anemia, at high altitudes, or due to lung disease. EPO binds to specific receptors on red blood cell precursor cells in the bone marrow, promoting their growth, maturation, and survival. The result is an increased number of circulating red blood cells and improved oxygen-carrying capacity of the blood.
Medical Relevance
Anemia in Chronic Kidney Disease
In patients with chronic kidney disease, the production of endogenous EPO is significantly reduced, leading to renal anemia. This condition causes fatigue, reduced physical performance, and shortness of breath. Recombinant EPO (rEPO), a synthetically manufactured form of the hormone, is used therapeutically to treat this type of anemia.
Anemia in Cancer Patients
Chemotherapy can also cause anemia. Recombinant EPO is used in these cases to stabilize hemoglobin levels and reduce the need for blood transfusions.

- Anemia in premature infants
- Preparation for major surgical procedures (autologous blood donation stimulation)
- Myelodysplastic syndrome (a bone marrow disorder)
Diagnosis: EPO Blood Levels
EPO levels can be measured in the blood. A low EPO level in the presence of anemia suggests a kidney-related cause. Elevated EPO levels may indicate secondary polycythemia (increased red blood cell production in response to oxygen deficiency) or, rarely, an EPO-secreting tumor.
Dosage and Administration
Recombinant erythropoietin is administered either as a subcutaneous injection (under the skin) or intravenously (into a vein). The dose depends on the current hemoglobin level, body weight, and treatment goals. Regular blood monitoring is recommended during therapy. Iron supplementation is often required alongside EPO treatment, as iron is essential for red blood cell production.
Side Effects
- High blood pressure (common, especially with rapid increases in hemoglobin)
- Thrombosis and thromboembolic events (increased risk when hemoglobin rises too quickly)
- Flu-like symptoms, headaches
- Rare: Pure Red Cell Aplasia (PRCA) – a serious suppression of red blood cell production caused by antibodies against EPO
EPO in Sports (Doping)
Due to its blood-boosting and performance-enhancing effects, erythropoietin is misused in endurance sports. The use of EPO for doping purposes is strictly prohibited and is detected through blood and urine tests. Excessively elevated hematocrit levels increase blood viscosity and can lead to life-threatening thrombosis.
