A functional test is a medical diagnostic procedure that evaluates the performance of an organ or organ system. It helps detect and assess diseases at an early stage.
What Is a Functional Test?
A functional test is a medical examination procedure used to assess the performance or functionality of a specific organ, organ system, or physiological process in the body. Unlike purely structural investigations such as X-rays or ultrasound, functional tests focus on how well an organ is actually working.
Functional tests are used across virtually all medical specialties – from internal medicine and pulmonology to endocrinology and neurology. They provide essential information about whether an organ is performing normally, at reduced capacity, or excessively.
Types of Functional Tests
There are various categories of functional tests depending on the organ or system being examined:
Pulmonary Function Tests
Pulmonary function tests, also known as spirometry, measure breathing capacity and airflow in the airways. They are used to diagnose conditions such as asthma, COPD (chronic obstructive pulmonary disease), and pulmonary fibrosis.
Liver Function Tests
Liver function tests are blood tests that measure enzymes and proteins produced by the liver. Key parameters include ALT (alanine aminotransferase), AST (aspartate aminotransferase), bilirubin, and albumin. They help identify liver conditions such as hepatitis, liver cirrhosis, or fatty liver disease.
Kidney Function Tests
These tests evaluate the filtration capacity of the kidneys. Important values include creatinine, urea, and the glomerular filtration rate (GFR). They indicate how effectively the kidneys remove waste products from the blood.
Thyroid Function Tests
Thyroid function tests measure hormone levels in the blood, particularly TSH (thyroid-stimulating hormone), fT3, and fT4. They are used to diagnose hypothyroidism (underactive thyroid) or hyperthyroidism (overactive thyroid).
Cardiac Function Tests
These include the ECG (electrocardiogram), stress ECG (ergometry), and echocardiography. They provide information about the electrical activity and pumping function of the heart.
Neurological Function Tests
Neurological functional tests assess the function of the nervous and muscular systems, for example through electromyography (EMG) or nerve conduction velocity (NCV) testing.
When Is a Functional Test Performed?
A functional test is used in the following situations:
- When an organ or systemic disease is suspected
- To monitor the progression of a known condition
- As part of routine health screenings or check-ups
- To evaluate the effectiveness of a treatment
- Before surgical procedures for risk assessment
How Is a Functional Test Performed?
The exact procedure depends on the specific test. Many functional tests require no special preparation, while others may require fasting beforehand, temporary discontinuation of certain medications, or physical exertion. The treating physician or medical professional will provide detailed instructions in advance.
Importance and Benefits
Functional tests allow a precise assessment of organ performance and contribute to the early detection of diseases before structural damage becomes visible. They are an indispensable tool in modern medicine and help guide individualized treatment decisions.
