What Are Bioidentical Hormones?
Bioidentical hormones are substances whose chemical structure is exactly the same as the hormones naturally produced by the human body. They are typically derived from plant sources such as soy or wild yam and are processed in a laboratory to be molecularly identical to endogenous hormones such as estrogen, progesterone, and testosterone. This distinguishes them from synthetic hormones, which have a slightly different chemical structure.
Areas of Application
Bioidentical hormones are primarily used in hormone replacement therapy (HRT), particularly for:
- Menopausal symptoms in women, such as hot flashes, sleep disturbances, and mood changes
- Estrogen deficiency following surgical removal of the ovaries
- Progesterone deficiency associated with premenstrual syndrome (PMS)
- Testosterone deficiency in men (andropause)
- Hormonal imbalances related to various medical conditions
Mechanism of Action
Because bioidentical hormones are structurally identical to the body's own hormones, they bind to the same hormone receptors as their natural counterparts. This allows a physiological response very similar to that of naturally produced hormones. The body recognizes and processes these hormones in the same way as endogenously produced hormones, which is theorized to result in better tolerability.
Forms and Administration
Bioidentical hormones are available in several forms:
- Transdermal patches or gels (for estrogen and testosterone)
- Capsules or tablets (e.g., micronized progesterone)
- Creams and lotions for topical application
- Injections or implants (less common)
Some bioidentical hormone preparations are available as standardized, approved pharmaceutical products (e.g., micronized progesterone). In addition, so-called compounded preparations are individually prepared by pharmacies according to a physician's prescription. The latter are subject to less rigorous approval standards and are viewed more critically by medical professional associations.

Therapy should be individually tailored, regularly monitored, and always coordinated with the treating physician. Self-medication is strongly discouraged.
