What is a Blood Group Antigen?
A blood group antigen is a molecule – typically a protein or carbohydrate – located on the surface of red blood cells (erythrocytes). These antigens are genetically determined and define a person's blood type. The immune system recognizes foreign antigens and can produce antibodies against them, which may trigger dangerous reactions during blood transfusions or organ transplants.
Blood Group Systems
There are more than 40 internationally recognized blood group systems. The most clinically significant include:
- ABO System: The most well-known blood group system. The A and B antigens on red blood cells determine blood types A, B, AB, and O. Individuals with blood type O carry neither A nor B antigens.
- Rhesus System (Rh): The second most important system. The Rhesus D antigen (RhD) is particularly relevant: people with this antigen are Rhesus-positive (Rh+), while those without it are Rhesus-negative (Rh-).
- Kell System: The Kell antigen (K) can also trigger antibody responses during transfusions and is significant in pregnancy.
- Duffy, Kidd, and MNS Systems: Additional systems that may become clinically relevant in cases of repeated transfusions or specific clinical scenarios.
Importance in Blood Transfusions
Before a blood transfusion is performed, blood typing and a crossmatch (compatibility test) must be carried out. If a patient receives blood carrying a foreign antigen against which they already have antibodies, a hemolytic transfusion reaction can occur – in which the transfused red blood cells are destroyed. This reaction can be life-threatening.
Importance in Pregnancy
Blood group antigens are particularly significant in cases of Rhesus incompatibility during pregnancy. If a mother is Rh-negative and her unborn child is Rh-positive, the maternal immune system may produce antibodies against the child's RhD antigen. In subsequent pregnancies, this can cause hemolytic disease of the newborn (erythroblastosis fetalis). To prevent this, Rh-negative mothers are given anti-D immunoglobulin prophylaxis.
Genetic Basis
The expression of blood group antigens is controlled by genes inherited from both parents. A person's blood type is therefore a stable characteristic that remains unchanged throughout life. Knowledge of blood group antigens is also important in forensic medicine and paternity testing.
Diagnosis
Blood group antigens are identified in the laboratory through serological testing. Blood samples are mixed with known antibody solutions (test sera). Clumping (agglutination) of the red blood cells indicates the presence of the corresponding antigen. Advanced molecular genetic methods allow accurate identification of rare blood group types.
