What is Cerebrospinal Fluid?
Cerebrospinal fluid (CSF) is a clear, colorless body fluid that surrounds and flows through the brain and spinal cord. It fills the ventricular system of the brain, the central canal of the spinal cord, and the subarachnoid space – the area between two of the three membranes (meninges) that enclose the central nervous system (CNS). In a healthy adult, approximately 100 to 150 milliliters of CSF circulate within the CNS at any given time, while 400 to 500 ml are produced and reabsorbed each day.
Production and Circulation
CSF is primarily produced by the choroid plexus, a specialized network of cells and blood vessels located within the brain ventricles. From the lateral ventricles, it flows through the third and fourth ventricles into the subarachnoid space, where it is eventually reabsorbed into the venous blood system via the arachnoid granulations. This continuous cycle maintains a stable composition and a regulated pressure within the skull.
Functions of CSF
- Mechanical cushioning: CSF acts as a shock absorber, protecting the brain and spinal cord from physical trauma and sudden movements.
- Nutrient supply: It transports glucose, electrolytes, and other essential nutrients to nerve cells.
- Waste removal: Metabolic waste products and potentially toxic substances are removed from the CNS via the CSF.
- Pressure regulation: CSF helps maintain and regulate intracranial pressure.
- Immune function: CSF contains immune cells and antibodies that help protect the nervous system from infections.
Composition of CSF
Healthy CSF is crystal clear and consists of more than 99% water. Other components include:
- Glucose (approximately 60–80% of blood glucose levels)
- Proteins (in very low concentrations, approximately 15–45 mg/dl)
- Electrolytes such as sodium, potassium, chloride, and calcium
- Very few white blood cells (leukocytes), normally up to 5 cells per microliter
Diagnostic Importance – Lumbar Puncture
Analysis of CSF is a cornerstone of neurological diagnostics. A lumbar puncture (spinal tap) is performed by inserting a needle between the lumbar vertebrae to collect a small sample of CSF, which is then examined in the laboratory. Changes in color, cell count, protein, or glucose levels can indicate a range of conditions:
- Bacterial meningitis: elevated white blood cell count, increased protein, decreased glucose
- Viral meningitis or encephalitis: mildly elevated cell count (predominantly lymphocytes), near-normal protein and glucose
- Multiple sclerosis: presence of oligoclonal bands (specific antibody patterns unique to the CNS)
- Subarachnoid hemorrhage: bloody or xanthochromic (yellowish) CSF
- Neuroborreliosis: inflammatory changes with elevated cell count and Borrelia-specific antibodies
Pathological Changes
Disruptions in CSF circulation or composition can cause or signal serious medical conditions. Hydrocephalus (excess fluid in the brain) develops when CSF drainage is obstructed, leading to a buildup of pressure within the skull. Symptoms of raised intracranial pressure can include severe headaches, visual disturbances, nausea, and in severe cases, loss of consciousness.
