What is Clopidogrel?
Clopidogrel is a prescription medication belonging to the class of antiplatelet agents (platelet aggregation inhibitors). It works by preventing blood platelets (thrombocytes) from clumping together and forming dangerous blood clots (thrombi). The drug is typically taken orally in tablet form.
Indications – When is Clopidogrel Used?
Clopidogrel is primarily prescribed for patients at elevated risk of cardiovascular events. Common indications include:
- Prevention of heart attack, stroke, and vascular death in patients with established cardiovascular disease
- Acute coronary syndrome (unstable angina, non-ST-elevation myocardial infarction)
- ST-elevation myocardial infarction (STEMI), often in combination with acetylsalicylic acid (aspirin)
- After coronary stent implantation to prevent stent thrombosis
- Peripheral arterial disease (PAD)
Mechanism of Action
Clopidogrel is a prodrug -- meaning it is pharmacologically inactive on its own and must first be converted into its active metabolite by the liver enzyme CYP2C19. The active metabolite then irreversibly binds to the ADP receptor P2Y12 on the surface of platelets. This permanently inhibits ADP-mediated platelet activation and aggregation. Because the binding is irreversible, the antiplatelet effect lasts for the entire lifespan of the affected platelets (approximately 7 to 10 days).
Dosage
The standard maintenance dose is 75 mg of clopidogrel once daily, taken orally. In certain situations, such as acute coronary syndrome, a higher loading dose of 300 to 600 mg may be given initially to achieve a faster onset of action. The exact dosage is always determined by the treating physician.
Side Effects
Like all medications, clopidogrel can cause side effects. The most common and clinically significant include:
- Bleeding: The most important risk, including gastrointestinal bleeding, nosebleeds, gum bleeding, and prolonged bleeding after injury
- Gastrointestinal complaints: nausea, diarrhea, abdominal pain
- Skin reactions: rash, itching
- Rare but serious: intracranial hemorrhage, thrombotic thrombocytopenic purpura (TTP)
Patients should consult a doctor immediately if they experience unusual bleeding or bruising.
Drug Interactions
Clopidogrel has several important interactions with other medications:
- Proton pump inhibitors (PPIs) such as omeprazole may inhibit the CYP2C19-mediated activation of clopidogrel and reduce its effectiveness
- Other anticoagulants and antiplatelet agents (e.g., warfarin, aspirin) increase the risk of bleeding
- Non-steroidal anti-inflammatory drugs (NSAIDs) also increase bleeding risk when combined with clopidogrel
- Genetic variants in the CYP2C19 gene can affect the metabolism of clopidogrel -- poor metabolizers may have reduced therapeutic response
Contraindications
Clopidogrel must not be used in patients with:
- Known hypersensitivity to the active substance
- Active pathological bleeding (e.g., peptic ulcer with bleeding, intracranial hemorrhage)
- Severe hepatic impairment
