The coffee enema became widely known in the 20th century through German physician Max Gerson, who incorporated it into his so-called Gerson Therapy as a claimed treatment for cancer and chronic diseases. Gerson theorized that rectal absorption of coffee could detoxify the liver and eliminate harmful substances from the body. These claims have not been substantiated by rigorous scientific research.
Claimed Effects and Uses
Proponents attribute a variety of effects to the coffee enema, including:
- Detoxification of the liver and colon
- Relief of constipation
- Increased energy and sense of well-being
- Support in cancer treatment (an unproven claim)
- Reduction of inflammation
It is important to note that none of these effects are sufficiently supported by scientific evidence. The human body has its own efficient detoxification systems, primarily the liver and kidneys, which do not require external support through enemas.
Proposed Mechanism of Action
Advocates claim that caffeine and palmitic acid in coffee are absorbed through the rectal mucosa into the portal circulation, where they are said to stimulate the production of glutathione S-transferase -- a natural detoxification enzyme -- in the liver. However, robust clinical studies confirming this mechanism in humans are largely absent.
Risks and Side Effects
Coffee enemas are not without risk. Known and documented adverse effects include:
- Electrolyte imbalances (e.g., hypokalemia, hyponatremia) with potentially life-threatening consequences
- Infections and bowel perforations due to improper administration
- Caffeine toxicity from rectal caffeine absorption
- Nausea, vomiting, and abdominal cramps
- Sepsis from damage to the intestinal lining
- Deaths related to coffee enemas have been reported in the medical literature
Individuals with bowel disorders, heart conditions, kidney disease, or a compromised immune system are at particular risk.
Medical Assessment
Conventional medicine and leading health organizations, including the World Health Organization (WHO) and the National Cancer Institute (NCI), consider the coffee enema to be ineffective and potentially harmful. No controlled clinical trials have demonstrated a therapeutic benefit of coffee enemas in humans. Physicians and pharmacists generally advise against this practice.

Individuals experiencing digestive issues, constipation, or a desire for internal cleansing should consider medically accepted approaches, such as:
