What is a CT Thorax?
A CT thorax (computed tomography of the chest) is a diagnostic imaging procedure that uses X-rays and a computer to generate detailed cross-sectional images of the chest. It allows precise evaluation of the lungs, heart, large blood vessels, esophagus, lymph nodes, and the bony structures of the chest wall (ribs, sternum, spine).
Compared to a standard chest X-ray, a CT thorax provides significantly more detailed, layered images, making it easier to detect even small abnormalities.
When is a CT Thorax Performed?
A chest CT scan is used for a wide variety of medical indications, including:
- Lung diseases: Detection or exclusion of pneumonia, pulmonary embolism, lung cancer, COPD, or pulmonary fibrosis
- Tumor diagnostics: Identification and assessment of tumors or metastases in the chest region
- Cardiac conditions: Examination of the heart and major vessels, e.g., suspected aortic aneurysm
- Trauma: Assessment of injuries after accidents (e.g., rib fractures, hemothorax)
- Infections and inflammation: Detection of abscesses, pleural effusions, or mediastinitis
- Pre-surgical planning: Preparation for thoracic surgical procedures
How is a CT Thorax Performed?
The patient lies on a table that slowly moves through a ring-shaped X-ray machine called a gantry. The scan itself typically takes only a few minutes. In many cases, an intravenous contrast agent is administered to better visualize blood vessels and certain tissue structures. During the scan, the patient is asked to hold their breath briefly to avoid motion artifacts caused by breathing.
Preparation
- If contrast agent is used: fasting for a few hours beforehand is usually required
- Information about kidney disease, allergies, or thyroid conditions is important (relevant for iodine-based contrast agents)
- Metallic objects (jewelry, underwired bras) should be removed
Contrast Agent
The commonly used iodine-based contrast agent is injected into a vein and improves the visualization of blood vessels and tumors. In rare cases, allergic reactions or impairment of kidney function may occur.
Radiation Exposure
A CT thorax involves a higher radiation dose than a conventional chest X-ray. The effective radiation dose is approximately 4 to 8 millisieverts (mSv), depending on the imaging protocol. Modern CT scanners and optimized protocols minimize radiation exposure as much as possible. The benefit-to-risk ratio is always carefully assessed by the physician.
Evaluation and Results
The generated cross-sectional images are evaluated by a radiologist and documented in a written report. This report is sent to the referring physician, who discusses the findings with the patient. Depending on the results, further examinations or treatments may be initiated.
Risks and Contraindications
- Pregnancy: CT thorax should only be performed during pregnancy in cases of urgent medical necessity
- Renal insufficiency: Impaired kidney function may limit the use of contrast agents
- Contrast agent allergy: Known allergy to contrast agents requires pre-treatment or alternative imaging
- Radiation exposure: Repeated CT scans should always be medically justified
