The liquid -- which may be a medication, a rinsing solution, or a diagnostic agent -- is delivered directly to the target site, bypassing the bloodstream and achieving local effects.
Areas of Application
Instillation is used across many medical specialties. The most common areas of application include:
- Eye drops (ocular instillation): Delivery of medications directly into the conjunctival sac of the eye, used for conditions such as glaucoma, conjunctivitis, or dry eye syndrome.
- Ear drops (auricular instillation): Introducing solutions into the ear canal, e.g., for ear infections or to soften earwax (cerumen).
- Bladder instillation (intravesical instillation): Introduction of medications directly into the urinary bladder via a catheter, commonly used in the treatment of bladder cancer (e.g., with chemotherapeutic agents such as Mitomycin C), interstitial cystitis, or recurrent urinary tract infections.
- Nasal instillation: Delivery of saline or decongestant solutions into the nasal cavity.
- Intrapleural instillation: Introduction of substances into the pleural space (between the lung and chest wall), used in cases of pleural effusion or for pleurodesis.
- Intraurethral instillation: Local treatment within the urethra, used in urology.
Procedure
The procedure for instillation varies depending on the target site. General principles include:
- The liquid is introduced slowly and in a controlled manner -- often drop by drop.
- Eye and ear instillations are often performed by the patient themselves or by medical staff.
- Bladder and body cavity instillations require sterile technique by trained healthcare professionals, typically using a catheter or specialized applicator device.
- The instilled liquid is intended to remain at the target site for a defined period before being drained or absorbed.
Advantages Over Systemic Administration
A key advantage of instillation over oral or intravenous drug administration is its local action: the medication acts directly on the affected tissue, significantly reducing systemic side effects (effects throughout the entire body). This is particularly important in bladder cancer chemotherapy, where high local concentrations of the active substance can be achieved with minimal systemic burden.
Risks and Side Effects
Although instillation is generally well tolerated, the following adverse effects may occur depending on the site of application and the substance used:
- Local irritation or burning sensation (e.g., in the eye or bladder)
- Allergic reactions to the instilled substance
- Risk of infection with invasive instillations (e.g., bladder instillation via catheter)
- Errors in application (e.g., incorrect dosage or poor technique with eye drops)
For invasive instillations, particularly bladder instillation, strict hygiene and sterile technique are essential to prevent infection.
